SELF-EVALUATION
QUALITY MANAGEMENT SYSTEM OF MEDICAL SERVICES (CSQMS)

1.Introduction
CSQMS designed to automate the process for the accreditation of health. Users work with the subsystem via a web portal, which is to enter the required user authentication.
The subsystem includes the following tasks:
- Formation of a package of documents;
- Registration of the application and documents to pass accreditation;
- Self-assessment of the medical organization;
- Review of the medical organization of the application;
- Carrying out an external evaluation of accredited entities;
- Management Accreditation Commission meetings;
- Printing and issuance of the certificate of accreditation;
- Maintain a register of accredited entities.

To work in the subsystem "Automation of the process for the accreditation of health" requires an Internet browser (we recommend Internet Explorer), Silverlight, Internet connection.
In the address line of the internet browser address entered opens CSQMS site, to enter which requires authorization. Users CSQMS are:
1. The certification of specialists departments, the territorial department of accreditation and licensing CCMPA MOH,
2. The employees of the Center of the National Accreditation Centre for Health Development,

The system displays the requirements of the standards selected section corresponding to the established criteria. Standards and description of the criteria are displayed by default. Fields "evaluation" and "justification of assessments" are required.
Evaluation - you must select one of the following:
- Full compliance with requirements of the standard;
- Partial compliance with the standard requirements;
- Non-compliance requirements of the standard;
- Not applicable.

After registration, evaluation data should be saved by clicking the "Save" button. Before conservation evaluation results via buttons can andedit the assessment and evaluation study. After saving the user can change the results of the evaluation standards passed by pressing the button "Edit" . It is also possible to register for the assessment of certain standards by selecting it via the reference standards that relate to Section.

After saving the system displays the results of the evaluation for each section. You must be assessed by type of assistance that has MO.
To view the results of the evaluation, namely "rating on the list", "Summary table estimates" and "self-assessment results," you want to search by clicking on the "Search" button and click on the button(Figure 15).

It is necessary to confirm the results of self-assessment in all sections. After confirmation of the necessary self-partition the user must complete the self-assessment by clicking the "Complete self-esteem". Thesystemprovidesinformationontheconfirmationmessage (Figure 16).

The user confirms the completion of self-assessment by clicking "Yes". The system saves the completion of self-assessment and self-assessment assigns a number (Figure 17).

After completing the self-assessment you must submit an application to the accrediting body (registration application described in section 5.3.1).
