Please explain your previous puppy raising or dog training experience:

Training a working dog means that you must regularly bring this dog into public places and situations where dogs are not normally permitted. Do you feel comfortable with this? Are you prepared for the attention that having a dog in public will bring?

Raising a puppy in preparation for it to become a working dog requires a lot of time and attention, more than would be needed to raise a puppy as a pet. Please briefly explain how you feel that your lifestyle would allow you the time to spend with this puppy.

Do you have any physical limitations that may affect your ability to train a dog? If yes, please explain:

Are you willing to sign an agreement that states that a member of WALK may, at any time, repossess the dog at their discretion?

Are you willing to sign an agreement that states you will contact WALK immediately if the dog requires veterinary care outside of routine care, and that failure to do so may result in you being financially responsible for unauthorized treatment?
