Prospective Puppy Parent Questionnaire
1. Have you ever owned a standard poodle? If yes, please describe past pets.
2. What made you decide to purchase a standard poodle?
3. Are you interested in a particular color or sex?
4. Are you aware of the time and monetary commitment concerning grooming for standard poodles, and do you have a groomer already or are you planning on grooming your pet yourself?
5. Is there someone home during the day?
6. Do you have children (if yes, are they familiar with dogs, and please list their ages)?
7. Does anyone in your home have allergies to animals (if yes please be aware that he/she can still be allergic to poodles, although this is generally less common)?
8. Do you have a fenced in back yard, and if not do you have a plan for exercising your puppy (please describe)?
9. Please list the town and state in which you live, and describe your home in general terms?
10. Do you have a plan for housing your puppy when you go away on vacation (please describe)?
11. Do you plan on attending obedience classes and/or any performance classes such as agility, flyball, etc.?
12. Do you have other pets (please list species and ages and whether or not they are familiar with dogs)?
13. A standard poodle can live 15- 16 years or longer, are you committed to a puppy for this length of time and throughout his/her lifetime for whatever illness, injury, etc. may occur (while I will always take back a puppy for any reason, as a veterinarian I see too many people who do not provide quality care for their pets, and it is very important to me that people are committed to any puppy they may adopt from me)?
14. My puppies are raised on a mix of raw foods (meats, bone, vegetables) and all-natural kibble. Have you heard of the “raw diet” natural feeding program, and are you willing to try it?
15. All of my pet puppies are sold with a spay/neuter agreement, meaning that you are required to spay or neuter the puppy around 6 months of age. Spaying and neutering are very important for a variety of reasons. Do you have any questions concerning this requirement?
16. If you are interested in a show puppy, please describe your experience in conformation showing, list any past champions, and explain why you are interested in a Tivin puppy for showing. While I very rarely sell a show puppy for a variety of reasons, if I do sell one it is with a co-ownership agreement until the puppy has completed his/her championship and genetic testing, after which I am willing to sign off. Do you have any questions concerning this practice or any other questions concerning show puppies?
17. I require references before selling any of my puppies, including a veterinary reference if possible. Can you please list 3 people with telephone numbers (including your veterinarian if possible) you would like me to contact as a reference?
18. Finally, please list any questions you may have that are not covered in this questionnaire or on my website. I would be happy to answer them.
