Work Week: Sunday – Saturday

Overtime Rate(s): 1.3 X’s above normal rate for all hours worked over 40 hours in a week

On-Call Rate(s): The provider will be paid $5.00 her hour. The provider is to carry a beeper or cell phone and remain within 30 minute radius of the facility while in on-call status. Phone consultation is included.

Call Back Rate(s): 1.3 X’s the hourly rate with a 1 hour minimum. This rate is charged once the provider begins working at the facility and has worked 40 hours in a week.
