- Shower furniture (showering, bathing & toiletting equipment)

- Prescribers are strongly encouraged to find a suitable product from the contract. Significant savings from contract items allow us to help more people, more quickly within the available budget.
- Please note that some of the contract items are available from stock, through the Equipment Allocation Program (EAP). The stock code is identified on the QRG, where applicable.
- Quotes are not required for contract or stock equipment. The relevant product code should be provided in the Equipment Request Form (ERF).

Which forms and Clinical Criteria do I use?
It is recommended that you do not save or print forms and other information as regular updates are made. Download the forms from the EnableNSW website and check the Clinical Criteria relevant to the type of equipment being requested.
If the person lives within the EAP delivery area, request the item from stock, by submitting a Stock Equipment Request Form (SERF) online -

If the most clinically appropriate product is not available on the government contract, EnableNSW will continue to provide non-contract items within the EnableNSW clinical criteria. Please provide reasons that a Contract item does not meet the person’s clinical needs and the outcome of any trials in the request form.
