Post Prom Party 2007 Parental Consent/Contract of Conduct

Dear Senior Parents and Seniors,
In keeping with the State of Illinois Department of Substance Abuse Guidelines, bags will be inspected for contraband (alcohol, drugs, tobacco products, weapons). Anything questionable will be removed, bagged and labeled for return to the student Monday morning.
Dress Code: The recommended attire might include t-shirts with shorts, jeans or sweats, and sneakers or sandals. We ask that there be no tank tops, crop tops, or short shorts.
Smoke Free, Alcohol Free, Drug Free: The Post Prom Party is a smoke free, alcohol free and drug free event for the gals, their dates and the chaperones. No one will be permitted to “step outside” for a quick smoke. Once the attendees are there, they may not leave.
In the event of an emergency the person listed as the emergency contact will be called, and asked to sign out and pick up the attendee with the emergency. Emergencies may consist of health concerns, behavioral problems, or intoxication to mention a few. What constitutes an emergency is at the discretion of the Post Prom Committee members present. The parents of the Seniors attending are responsible for their son’s/daughter date in an emergency.
Return of attached form: Please review this information and sign the attached form designating you have read and accept the terms of attendance at Post Prom. We ask that if your daughter does not plan to attend the Post Prom that you please sign the attached form and mark the appropriate box, so that we may plan accordingly.
NO ONE MAY ATTEND THE POST PROM WITHOUT A SIGNED CONSENT FORM ON FILE.

It is our understanding we are responsible for arranging transportation of our daughter and her guest to the Prom;
Our sons/daughters and their dates may not leave Post Prom until the conclusion of the event;
There is to be no use of alcohol, tobacco or drugs by those attending;
If there is an emergency, we may be contacted at the numbers listed below;
Emergencies are not limited to health concerns, but may include behavioral problems or intoxication (as determined by the Post Prom Committee);
Failure to follow standard rules of conduct, or abide by the decisions or direction of the chaperones may constitute an emergency;
My son/daughter and his/her date are responsible for their own actions, and I may be called if either creates an emergency;
We agree to hold WinchesterHigh School and the Post Prom Committee and their agents harmless for any liability in connection with the Post Prom Party.
