Position Description
POSITION TITLE: Behavior Health Integration (BHI) Case Manager
DUTIES AND RESPOSIBILITIES:
- Conducts case management and treatment services according to ECHO Community Health Care Policies.
- Provides case management services to patients assigned by the primary medical integration team, including: communicating with all parties involved in the patient’s treatment; identifying treatment needs and facilitating all necessary referrals; and coordinating collateral meetings with all parties involved in the treatment process; driving patients to and from appointments; performing in-home assessments and visits.
- Provides emergency crisis intervention for own caseload when necessary.
- Maintains appropriate records of services to reflect patient progress, documents this information in the electronic medical record, and submits reports according to established time frames
- Communicates professionally, make necessary referrals, and coordinates services with community agencies.
- Participates in ECHC’s committees, staff meetings, required education activities, mission directives and continuous quality improvement processes.
- Accesses electronic medical records to view, create, or modify information.
- Complies with all confidentiality requirements, which are mandated by law and apply to all staff, including volunteers
REPORTS TO: BHI Project Coordinator
SUPERVISES: None.
LICENSURE: Current, valid state driver’s license.
QUALIFICATIONS: Bachelor’s degree inSocial Work or a related human services field from an accredited university or college required.
SKILLS: The BHI Case Manager must be able to function in team settings where lines of authority and job delineations overlap, be able to work in collaboration with other disciplines and ECHC teams, and demonstrate the ability to communicate effectively and professionally with staff, clients, physicians, peers, supervisors, and other collaborative community agencies. This individual must show concern for client’s dignity and participate in the promotion of ECHC’s mission.
KNOWLEDGE: The BHI Case Manager must be familiar with mental health and substance abuse basic treatment issues, treatment planning procedures, and documentation with an electronic medical record, as well as be able to use Windows applications.
EXPERIENCE: The BHI Case Manager will haveone to two years of community or social service experience. A background with people experiencing mental health issues and substance abuse challenges is required..
TRAVEL REQUIREMENTS: Localtravel is required for services provided at multiple sites. Also, there is some travel for seminars and continuing education.
SALARY RANGE, WORK HOURS, AND JOB CLASSIFICATION: Full time – 1 FTE with the following hours: generally Monday through Friday 8am-5pm, with a one hour lunch.
