Contractors Operations

Contract start date is June 2, 2008. Contract completion date is September 20th.

Aggregate base is scheduled during the week of July 21st on CSAH 21 and 66. Paving is planned for August 4th. Normal work hours will be Monday thru Friday and Saturday as needed for makeup days.

Utilities

No Utilities affected by this project.

Safety

The Contractor has an on-going safety program with weekly safety meetings and daily equipment checks. The contractors safety officer is Tony Brevig. It will be the Contractor's responsibility to maintain traffic control devices within the project limits. The Contractor is required to check the construction signing daily. Safety vests and other safety equipment shall be worn at all times while on the right of way. Polk County's Safety Representative is Darin Carlstrom.

Labor Provisions

This contract is subject to state prevailing wage laws.

The Contractor was given the required notices on wages, nondiscrimination, EEO, and Storm Water Posters, which shall be posted on the job site. Certified payroll shall be submitted by all contractors and trucking sub-contractors. Trucking subs shall submit monthly trucking reports. Knife River materials should make all subs aware of the prevailing wages. Monthly interviews will be conducted as required for wage compliance.

Erosion Control

The contractors shall monitor potential soil erosion areas within the project limits. Berms, dikes, dams, silt fence, retention ponds, and bale checks shall be used where needed to control erosion. Polk County will closely monitor all sensitive areas.

Engineers Comments

1. The Project Engineer on the job will be Darin Carlstrom.

2. Construction signing shall be inplace prior to any work.

3. Partial payments will be the first Tuesday of the month with the cut-off date being approximately the end of the third week of the month prior to payment. Other payments may be made as requested.

4. It is understood by both parties that changes/modifications that arise in the field that differ from our plans will be discussed and resolved before work is started.

5. An open line of communication is important by all parties. Problems, which arise, should be

handled up front and not later.

6. The Bituminous Aggregate shall have a CAA of 55% for one face.

7. Required forms: Certification of materials form and Request to Sublet.

8. Railroad Insurance is required for CSAH 21 and CSAH 23.
