By signing this entry form, I declare that I am familiar with the current requirements and procedures regarding therapeutic use exemptions (tue) and doping controls and with the present anti-doping-rules and regulations determined by IAAF as well as the anti-doping-codes issued by WADA and NADA and that I acknowledge them to their full extent. I further declare and affirm herewith to abstain from any illegal performance enhancement, specifically by way of doping and to support anything that prevents the use of unintended or extraneous doping medication. Prohibited substances and methods are listed in the 2012 prohibited list issued by WADA and, as in the 2013 list. I know that as a participant in this competition I am subject to doping control and that I am obliged to submit myself to doping control if asked to do so. I know that refusal to submit to doping control as well as a positive result may lead to sanctions up to suspension.

Mit meiner Unterschrift erkläre ich, dass ich von den aktuellen Anforderungen und Verfahren im Zusammenhang mit medizinischen Ausnahmegenehmigungen (TUE) und Dopingkontrollen sowie den derzeit geltenden Anti-Doping-Bestimmungen (DLV-ADC, IAAF-Anti-Doping-Rules, NADA-Code, WADA-Code, WMA-Rules and Regulations) Kenntnis genommen habe und diese Regeln vollumfänglich als für mich verbindlich anerkenne. Ich erkläre, jegliche unerlaubte Leistungsmanipulation, insbesondere durch Doping, zu unterlassen und alles zu tun, um eine unbeabsichtigte verbotene Medikation zu verhindern. Aktuell verbotene Substanzen und Methoden sind der WADA-Verbotsliste 2012 und ab dem 01.01.2013 der WADA-Verbotsliste 2013 zu entnehmen. Ich weiß, dass ich bei diesem Wettkampf zu einer Dopingkontrolle aufgefordert werden kann und verpflichtet bin, mich dieser Kontrolle zu unterwerfen. Ich weiß, dass sowohl die Verweigerung der Dopingkontrolle als auch ein positiver Befund Sanktionierungen bis hin zu einer Wettkampfsperre nach sich ziehen können.
