R u clear Screening – Sexual Health
CHLAMYDIA. The most common STI in people under 25. Chlamydia often has no symptoms. Finding out if you have chlamydia is simple – just one quick and painless test that you do yourself. You do NOT need to be examined! It is simply a urine test which tests for Chlamydia and Gonorrhoea. As a practice it is vital for us to ask any of our patients aged 14 - 24 if they would like the screening. If you would like to discuss this privately, please ask the Reception Team, who will then explain further in a consultation room.

Please keep us updated with your contact details i.e. contact number, address and next of kin. This is important for all patients and staff.Please fill in a UPDATE PATIENT RECORDS form located on reception

PRESCRIPTIONS
You should have 2 days medication left before ordering prescriptions, as it takes 48 working hours/ 2 WORKING DAYS for it to be signed and ready. We CANNOT accept requests over the telephone. All requests MUST be ordered in person at the surgery OR via your local pharmacy.

  • Breast screening
You will be invited to participate if you meet the criteria. Please attend these appointments as they are very important as they could detect early signs of serious disease.

Patients aged 40-74 will be invited for a health check. The health check involves:
  • Measuring your blood pressure
  • Measuring your height and weight
  • A simple blood test to measure your cholesterol and sugar levels
  • Finding out about your family history
If you are within the age criteria please feel free to book a NHS OVER 40’S Health Check with the Health Trainer.

Do you look after someone or does someone look after you?
Evidence shows that if the carer becomes ill, the likelihood is that the patient cared for is affected in somewhere.
Please ask at the reception for a Carers Form so that we can record your details.
Carers registered at this practice will be invited to attend a Carers Review appointment.

Annual Reviews
If you suffer from the following;
COPD, Asthma, Heart Failure, Diabetes or Hypertension – You will need to check and book an Annual Review with the PRACTICE NURSE.
COPD
If you have COPD and require regular inhalers then you should have a review of your medication at least once a year.
Hypertension
Many people have high blood pressure (hypertension) and take medication to try and keep it at the required level. If you have hypertension then it is important that you use the blood pressure machine at the surgery when requested.
Heart Monitoring
We offer an annual appointment to patients with established heart disease.
The purpose of this clinic is to support you in managing your heart condition and to ensure that the quality of your health remains at its best.
Asthma
If you have asthma and require regular inhalers then you should have a review of your medication at least once a year.
