MEDICAL/ DENTAL SCREENING CHECKLIST: REQUIRED SUPPORTING MEDICAL DOCUMENTATION MUST BE COMPLETED AND BROUGHT TO MARINES SECURITY GUARD SCHOOL
********PLEASE NOTE A WORLDWIDE SCREENING IS REQUIRED TO BEGIN TRAINING*********
Medical Screening:
1. DD 2807/2808: specifically stating “FIT FOR DEPLOYMENT IN AUSTERE/REMOTE LOCATION”
2. Original medical records
3. Copies of civilian medical records , if any
4. G-6PD documented
5. Sickle cell testing documented
6. DNA documented
7. Audiogram documented within six months of reporting
8. Blood typing completed
9. Allergies documented including any food allergies - If “yes”, red dog tags will be issued
10. HIV / lipids within six months of reporting
11. Flu and TDAP documented
12. PPD within 90 days of entering country
Dental Screening:
1. Original dental records
2. Must be a Dental Class 1. This classification should reflect a T-2 dental examination
within 180 days of reporting date.
Optometry:
1. Comprehensive eye exam including dilation within 2 years
2. No eye surgeries six months prior to attending Marine Security Guard School
Up to date Immunization Status:
1. Hepatitis A and B or TWINRIX series complete
2. IPV adult dose
3. Yellow fever (10 years)
4. Documented MMR and VZV recruit panel showing positive antibodies
5. Malaria (Ensure SNM/Family Member is capable to take Malaria)
*****SNCO’s with family members must bring a copy of all family member’s medical records in a sealed envelope to MSG school*******
1. Family members whether seen by a civilian or military provider (to include specialties) must complete the following forms along with 1-2 years of medical records:

- Signed 2792-1 (School age children only)
- Signed 2807

- Signed 2808
