We want to identify appropriate development-centered linkages for a family with a child who screened positive, has a developmental concern, or home visitor concern using 2-1-1?

We will have identified appropriate development-centered resources through 2-1-1 and connected families to them during a home visit.
  1. What ideas do we have that will result in improvement (change to test)?
During a home visit, one home visitor willlink a familywith a child who screened positive, have a child developmental concern, or home visitor concern to resources centered on developmental promotion available through 2-1-1.

What did you observe that wasn’t part of the plan? Nothing
Study / What did the data tell you? Family declined linkage to development-centered resources potentially available through 2-1-1. Instead, family requested to be linked to concrete services.
What surprised you?Family feels that receiving a combination of EI services and home visitor promotion is sufficient for their children’s development, for the time being.
