Performance Review Instructions and Forms

The performance review process involves eight steps:

1. The supervisor forwards a copy of the Performance Review Form to the employee along with the employee’s current position description.

2. The supervisor and the employee complete Part I (questions 1-3) on their respective forms.

3. The responses on Part I (questions 1-3) are exchanged between supervisor and the employee.

4. The supervisor and the employee complete Part II, question 4, on their respective forms.

5. The supervisor meets with the employee to review the employee’s performance. The supervisor and the employee discuss the responses on each review form and add comments as necessary.

6. The supervisor and the employee complete Part II, question 5.

7. The supervisor and employee sign the completed Performance Review Form. If appropriate, the supervisor sends the review forms to the next level (Chair and/or Co-Chairs).

8. The position description is revised, initialed, and dated.

2. Job Performance:

a. List aspects of the employee’s duties that were performed particularly well

this past review period.

b. List work-related accomplishments and contributions that the employee

completed since the last review including one-time special projects and

assignments.

c. Indicate an aspect or aspects of the employee’s performance where

improvement is possible.

3. The employee should be working on the following objectives for the coming year. Indicate what would be particularly helpful in meeting these goals (i.e., training programs, courses, etc.).

Part II

4. SUPERVISOR’S COMMENTS about the written Performance Review Form completed by the employee.

5. SUPERVISOR’S COMMENTS following discussion of performance review.

The signatures below do not necessarily indicate agreement, but indicate that the employee and supervisor have had an opportunity to read and discuss this review.
