I authorize Foster Care Services personnel to access/modify my account with PDS Online for the purpose of managing my training log. I have been provided with my user ID and password (above) and understand that I can access/modify information on PDS Online at any time.

I do NOT authorize Foster Care Services personnel to access/modify my account with PDS Online for the purpose of managing my training log. I understand that by denying authorization I am solely responsible for managing my training log, and that failure to do so may result in corrective action.
