Agreement, Release and Indemnification for Daytime Socialization and/or Drop-off Dog Training Services

By signing this Agreement, Release and Indemnification (hereinafter the “Agreement”)and in consideration of services provided to you and/or to your dog(s) by Paw Law Dog Training Academy, (hereinafter,” PLDTA”) you, hereinafter, the “Owner”, acknowledges that you have paid an enrollment fee for your dog(s) to use PLDTA for daytime socialization (doggie daycare) and/or drop-off dog training, (hereinafter “Daytime Services”). Owner agrees that any and all fees are non-refundable and are not transferable to any other dog owner, who has not executed this Agreement. Owner acknowledges that on the date this Agreement is signed, the fees, hours and requirements for use of PLDTA’s daytime services are outlined below and on any attachments that are attached hereto. Owner understands that PLDTA’s management reserves the right to alter or amend the fees, hours and requirements at any time and with notice to the Owner.

1. Use of daytime services are to be provided only to dogs meeting the enrollment requirements of PLDTA as set forth in the Enrollment Requirements attached hereto.

2. Owner warrants that his/her dog(s) is healthy and has provided a certificate of health from a licensed veterinarian certifying them free of worms, parasites, fleas, ticks or any other communicable disease.

3. Reservations are required for all of PLDTA’s Daytime Services. All reservations must be made by noon the day prior to a dog attending any of PLDTA’s Daytime Services. Due to limited enrollment in PLDTA’s Daytime Services, any and all cancellations made after 4:00pm the day prior to a dog’s reservation, will result in the Owner being charged in full for the services reserved.

4. Fees are due and payable at time services are rendered. Daytime socialization hours and fees are:

$36.00 per full day Monday through Friday 6:00 am – 6:00 pm.

$27.00 per half day Monday through Friday for up to 5 hours.

Multiple Full & Half Day Passes are also available at a reduced rate.

Drop-off Training 8 Day Pass is $425 and Half-Day Pass is $310.

5. Suggested Drop-off times are from 7:00am to 9:00am for full day and morning only clients and from 1:30pm to 2:00pm for afternoon half-day only clients. Suggested Pick-ups are from 11:30-noon for morning half-day only clients. All dogs must be picked up 6:00pm. Any dogs picked up after 6:00 will be crated until the owner’s arrival and will be charged a late fee of $5.00 for every 5 minutes. Clients can arrange for drop-offs & pick-ups during non-business hours for a $15 fee.

OWNER AGREES

1. By executing this agreement, the undersigned, the Owner is/are solely responsible for any harm caused by their dog(s) while their dog(s) is/are attending PLDTA’s Daytime Services

2. That in enrolling their dog(s) inPLDTA’s Daytime Services, the Owner acknowledges that PLDTA has relied on the Owner’s representation that his/her dog(s) is/are in good health, does not currently have and has not had within the last fourteen (14) days any communicable diseases, and has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

3. Aggressive dogs are prohibited from all PLDTA’s Daytime Services programs. Definition of aggressive dog is – any dog that shows an inappropriate challenge, threat or contest to another being (human or canine). Owners and dogs asked to leave any of PLDTA’s Daytime Services because of their dog’s aggressive behavior, will not receive a refund of any fees paid. However, a pro rata credit may be applied for private sessions

4. That any problem, including a medical emergency, that develops with an Owner’s dog(s) will be treated as deemed best by owners, employees, and/or agents of PLDTA in their sole discretion,

and that the Owner assumes full financial responsibility for any and all expenses involved, including expenses resulting from injuries caused by his/her dog(s).

5. To follow all policies of PLDTA described in this agreement.

6. To pay for all services rendered and fees assessed as described in this Agreement.

7. To indemnify and hold harmless Paw Law Dog Training Academy and/or its officers, its employees, volunteers, instructors and/or any other person associated with Paw Law Dog Training Academy from any and all claims for personal injury and/or property damage of any member of the Owner’s family and/or other person accompanying the Owner to any training session or function on PLDTA’s premises, property or surrounding area thereto, including costs of defense.

8.To discharge Paw Law Dog Training Academy, its officers, employees, volunteers, instructors and/or any other person associated with Paw Law Dog Training Academy from any and all liability arising out of or in any way connected or associated with any accident and/or injury that may arise from said training or attendance of any and all functions at Paw Law Dog Training Academy even though that liability may arise allegedly or actually from any actions whatsoever of Paw Law Dog Training Academy or persons or entities acting on Paw Law Dog Training Academy’s behalf.

9. To hold harmless from any cost and/or attorney’s fees Paw Law Dog Training Academy or it’s officers, its employees, volunteers, instructors and/or any other person associated with Paw Law Dog Training Academy from any claim based on the Owner’s negligent use of the property or claims arising through the ownership of the Owner’s dog(s). The Owner(s) agrees to abide by any and all “house rules” set forth by Paw Law Dog Training Academy. This Agreement, Release and Indemnification supercedes all previous such agreements with Paw Law Dog Training Academy

Although we watch the dogs carefully, and DO NOT accept aggressive dogs, daycare can be hazardous due to multiple dogs playing together. We cannot be held responsible for injuries occurring while the dogs are playing.
