1. / Three members of staff who have completed Care Navigator training came along to the meeting to provide a progress report. There are currently around 17 patients in their task group, 5 of whom have been helped successfully.
One patient was having problems with a noisy neighbour which affected his/her mental health. Our CN contacted the local authority and the situation has been successfully dealt with.
The wife of a stroke patient contacted the practice asking if there was any support available for her husband who was now paralysed and could no longer speak. She was signposted to the stroke association who have provided excellent support and improved this patient’s quality of life.
Patients who suffer from anxiety/isolation have been signposted to support groups and one person now regularly attends the gym as well as a support group located at Wallsend library. These are a small example of how CN can improve quality of life for patients who need support.
Those present were advised that the Council “Mycare” website is now up and running. / Attend next meeting to provide update.

3. / Patient Partner will be installed on 17th August, some two months later than planned. Simplistically, the automated system will allow patients 24/7 telephone access to the practice. / Darren to meet with partners and staff to plan how this will be implemented in the practice.

4. / Darren has replaced Cynthia as Practice Manager. The group sought assurances that the present format of the meetings would not change too much and they were assured things would go forward in much the same way. / Circulate list of patient group successes.
5. / The group were not aware that NHS 111 have direct access to 7 telephone slots at the practice each day which they are able to book into for practice patients who contact them with acute problems which can be dealt with by the GP.
The location of the urgent care centre has not yet been agreed as one of the interested parties has made a legal challenge.
North Shields locality practices are in discussions as to how collectively they will run an out of hours hub. Primary care will be available until 8.00 pm each evening Monday-Thursday and Saturday morning. Appointments will be pre booked, patients should use urgent care centre for urgent care. One hub in the whole of North Tyneside may pilot Sunday access although this has not been decided. / Progress report to next meeting
6. / Mr JB has decided to resign from the group to spend more time at his holiday home in Spain. The group wish him well. We have a new member Mrs JD and welcome her into group.
