PAST YEAR’S GOALS
- The church objectives for the year as they applied to your ministry team, Objective 1
- Objective 2
- Objective 3 (Example: embedding church in community)*Make note of the rubric on second page.
WORK DONE TOWARD THE GOALS
- The basic strategy of your ministry team to meet the objectives: Objective 1: Proposed Actions 1,2,3
- Objective 2: Proposed Actions 1,2,3
- Objective 3: Proposed Actions 1,2,3 (Example: Offer more events in the community)
APPRAISAL OF THE WORK
- An honest look at how well actionsteps helped reach their objective.
- What was learned from the attempt?
- What you would repeat or change the next time.
(Example: Holding the garage sale at a residence instead of at the church was more successful. We had better connection in the community and raised more funds. However, in the next attempt, we will offer different types of food…) *Make note of the rubric on second page.
PROJECTED GOALS FOR NEXT YEAR
- There should bridge from last annual period to the coming year.
- Pastor should have out a basic letter to offer basic direction.
- Use 3 Objectives with 3 Action Steps as your method. *Make note of the rubric on second page.
SPECIAL THANKS
To anyone who was of particular help; people within the church and outside, if that applies. This helps us all know where our partnering is coming from. If there is no one in particular, this can be omitted.
(Example: Special thanks to Kids Count and the POA for allowing us the opportunity to engage the community…)

RUBRIC FOR OBJECTIVES AND ACTION STEPS
This page is for your own evaluation and doesn’t need to be submitted with your report. It is only as useful as the honesty with which it is answered.
OBJECTIVES:
- This objective is from God and not simply our own idea –
- This objective will direct people to Christ-
- This objective will impact people with the Gospel message-
- This objective is the product of persistent prayer/ meditation-
- This objective can be supported from Scripture (Have a verse in mind)-
- This objective comes from the movement of the Holy Spirit-
- This objective is in alignment with overall church objectives-
ACTION STEPS/ APPRAISAL OF ACTION STEPS
- This action step is from God and not simply our own idea-
- This action step helps meet a stated Biblical objective-
- This action step conforms to Biblical ethics-
- This action step impactspeople with Christ-
- This action step is in accordance with church governance-
- This action step does not isolate other members of the church family-
- This action step aids in mentoring and/or plugging in other members-
The purpose in quantifying our strategy is to guard against mere “church busy-ness”, and do all we can to ensure we are doing the work God wills, where God is, and in God’s way.
The math is simple;highest possible score is +7, lowest -7; anything not well in the positive is most likely not where God is working around us at this time.
Of course, it is not practical to quantify everything in numbers; however, nothing should be pursued unless it has been filtered through these values.
