Gofal has been providing mental health and wellbeing services to people in Wales since 1990. We have come a long way since our first services opened in Bridgend, supporting the resettlement of people from the long stay psychiatric wards of Parc and Penyfai hospitals into homes in the community. Part Time Case Worker

Location:Blackwood office base plus various community settings in Gwent

Salary:£18,685pro rata per annum.

Contract:12 month contract to June 2017

Hours:18.5 hours per week.

Holidays:26 days annual leave plus bank holidays.

Holidays increase by 1 day for each year of service up to a maximum of 30 days.

Pension:Pension in the form of a money purchase pension scheme:

-4.5% non-contributory employer contribution

-5.5% paid if employee contributes 1%

-6.5% paid if employee contributes 2% of more

The Service:Pathways to Employment provides a specialist service to people with mental health needs, assisting them to access or retain employment, access education and vocational opportunities, or re-discover skills and abilities that they may have lost. We do this by identifying and pursuing relevant training and offering support and mentoring to individuals relating to work opportunities. Using the Workstar Outcome model, we focus on aspirations, strengths, experience and the re-learning or development of new skills. To support this work, we have developed a number of positive relationships with employers and training providers to ensure that we can meet individual needs and support people on their journey of recovery.

Job Purpose

  • To directly deliver employment related support to people
  • To provide social, emotional and practical support to people with mental health and associated problems.
  • To comply with and maintain all necessary administrative and reporting systems.
  • To build links and opportunities with other work based organisations.

Role Expectations:

Support Work

  1. To carry out work star assessments with service users to identify support needs and develop a personalised support plan with jointly identified outcomes..
  2. To effectively manage and maintain one’s own case load to ensure that an effective service is delivered to service users.
  3. To provide information to, and work with Case Workers/ Line Management in reviewing progress and support plans.
  4. To regularly carry out and review risk assessments as and when necessary due to the changing needs of the service user.
  5. To provide a range of social, emotional and practical support as outlined in support plans.
  6. To meet the needs of service users in group and individual settings.
  7. To support service users to access information and advice in order that they are able to make informed choices in matters that affect them.
  8. To develop opportunities with organisations and businesses to support individual’s in achieving their aspirations and goals.

Team Working

  1. To seek advice and support from Line Manager as necessary in delivery of support.
  2. To assist in the development of specific work based skills groups such as CV writing.
  3. To liaise with other agencies with and on behalf of service users as part of planned support.
  4. To attend and participate in team meeting.
  5. To attend and participate in local community events to promote the service.

Administrative Requirements

  1. To keep accurate notes of all support provided.
  2. To ensure all information is kept up to date and accurate.
  3. To ensure all relevant information is shared with colleagues.
  4. To ensure all project communication methods are used appropriate and effectively.
  5. To ensure that IT systems are used effectively and in line with organisational procedures – e.g. Use of e-mail and database systems.
  6. To provide all relevant information to head office and external organisations as required.
