Parents As Teachers Measurable Indicators
*You may change only the numbers in red. Required state level measures are noted. Remaining indicators are SSHC required measures.
- At least X families will participate in the Parent As Teachers program
- At least X parents/guardians will participate in home visits through PAT (NCPC Mandatory Output)
- At least X children will be served through the Parents as Teachers Program
- X% (X of X) of families will receive at least 75% of their offered number of visits.
- A total of X group connections will be offered (NCPC Mandatory Output)
- X% (X of X) of families will participate in PAT group connections (NCPC Mandatory Output)
- X% (X of X) of children will receive a complete (developmental, social-emotional, hearing, vision, health) during the current fiscal year
- X% (X of X) of families will be referred and linked to at least one community resource
Comments:
The following indicators do not require an estimate. These are the indicators, taken directly from the LSP tool, that you must report to SSHC quarterly:
- 100% of parents/guardians enrolled in the Parents As Teachers program will have a PRE Life Skills Progression assessment completed (this indicator CANNOT be changed).
- At least 50% of parents/guardians enrolled in the Parents As Teachers program will have a POST Life Skills Progression assessment completed during the fiscal year (this indicator CANNOT be changed).

Post Scores (NCPC Mandatory Outcomes)
