Parent/Guardian Video Permission Form
Dear Parent or Guardian,
Your child is currently in Mrs. Hughes Modern World History class. It is my intention to show movies/videos or segments of movies/videos, which will help the students gain greater understanding of events which helped to shape our society and our world. These videos may or may not be shown.
Movies/videos include:
· All Quiet on the Western Front (UR) - A young soldier faces profound disillusionment in the soul-destroying horror of World War I.
· Band of Brothers (TV) - The story of Easy Company of the US Army 101st Airborne division and their mission in WWII Europe from Operation Overlord through V-J Day.
· Lord of the Flies (R or NR 1963) - Shipwrecked on an island, the castaway boys eventually revert to savagery despite the few rational kids' attempts to prevent that.
· Luther (PG-13) - During the early 16th Century idealistic German monk Martin Luther, disgusted by the materialism in the church, begins the dialogue that will lead to the Protestant Reformation.
· Oliver Twist (PG-13) – A classic Dickens tale, where an orphan meets a pickpocket on the streets of London. From there, he joins a household of boys who are trained to steal for their master.
· Rabbit Proof Fence (TV PG) - In 1931, three aboriginal girls escape after being plucked from their homes to be trained as domestic staff and set off on a trek across the Outback.
· Red Dawn (PG-13) - It is the dawn of World War III. In mid-western America, a group of teenagers bands together to defend their town, and their country, from invading Soviet forces.
· Saving Private Ryan (R) - Following the Normandy Landings, a group of US soldiers go behind enemy lines to retrieve a paratrooper whose brothers have been killed in action.
· Schindler’s List (R) - In Poland during World War II, Oskar Schindler gradually becomes concerned for his Jewish workforce after witnessing their persecution by the Nazis.
· The Hunt for Red October (PG) - In 1984, the USSR's best submarine captain in their newest sub violates orders and heads for the USA. Is he trying to defect, or to start a war?
· The Lost Battalion (TV-14) - Fact-based war drama about an American battalion of over 500 men which gets trapped behind enemy lines in the Argonne Forest in October 1918 France during the closing weeks of World War I.
· The Pianist (R) - A Polish Jewish musician struggles to survive the destruction of the Warsaw ghetto of World War II.
· World at War (TV) - A multi-volume documentary mini-series, covers the entire history of World War II from the causes of the 1920s to the aftermath of the Cold War in the 1950s.
This letter is being sent to you in compliance with the District policy requiring parents/guardians to approve the intended use of videotapes that require positive parent permission prior to their scheduled showing. As part of the policy, I am asking you to complete the form below, authorizing or exempting your child from the videotape showing. Students exempted from this showing will be required to complete alternative assignments.
