Michael Cropp, MD, Board Chair of the P2 Collaborative of Western New York convened the meeting at approximately 8:40am.

Updates

2008 P2 Conference – The Second Annual P2 Conference was held September 24th and September 25th. Approximately 330 people attended. The date for next year’s conference will be announced at the next quarterly meeting. Topics currently under consideration for the focus of next year’s conference include but are not limited to, disparities, healthy lifestyles, and cultural implications in health care.

RWJF AF4Q Hospital Collaboratives –The Robert Wood Johnson Foundation recently sent letters out to hospitals in the 14 Aligning Forces for Quality communities regarding the Transforming Care at the Bedside (TCAB) RFP. The goal of this endeavor is to increase the quality of care, increase the retention of nurses, and increase the effectiveness of the nurses. The deadline for consideration is December 10. The first program begins in February 2009 and requires a 3 year commitment from the hospital. The second round will begin in September 2009.

Collaborative Updates

Performance Measurement –Raghu Ram, MD was present to give updates on the Western New York Quality Measurement Collaborative. Quality measures data has been extracted from the health care plans and delivered to the data aggregator, ViPS. ViPS is currently creating reports which will be delivered to physicians by mid- to late January 2009. Members of the WNYQMC have spent a great deal of time discussing and deciding the manner in which patient care is attributed to particular physicians. A presentation has been created to reach out to explain the background and goals of the project and to actively engage physicians in utilizing the reports.

Regional Quality Improvement –Amber Slichta from the Community Health Foundation of Western & Central New York reported on the Quality Improvement strategy that will be launched the first quarter 2009. The first course will be a two day basic quality improvement training session for acute, long term, and ambulatory care providers. Follow-up training will be provided. On December 15, the final Reaching for Excellence Speaker Series will convene. The guest speakers will present their work on disparities in health care.

Consumer Engagement

Updates – Consumer engagement updates were reported by Tracy Sawicki, the P2 Collaborative of Western New York Consumer Engagement Team Leader.

  • CELC Meeting in New Orleans- The Consumer Engagement leadership teams from the 14 Aligning Forces for Quality communities recently met in New Orleans and discussed how to best present messages. The partnership between the P2 Collaborative and WNED puts the P2 Collaborative ahead of the curve. WNED, in their “My Health Counts” series is utilizing the messages from the Robert Wood Johnson Foundation which have been tested over time.
  • Stanford Chronic Care Model – sends trainers out into the community to promote self-management of chronic diseases. Members of the community are being recruited (Block Club Leaders, BuffState, physicians’ offices) to push educating consumers on the importance of self-management. PEA and CEA pilot programs are being utilized in the community to educate consumers and physicians.
  • Diabetes Conversation Map Training – hosted by Merck and the ADA, will be taking place on December 2, 2008 at the Adams Mark. The Diabetes Resource Guide has one last tweak and then it goes to print.

Consumer Survey: Aligning Forces Initiative – Jeff Beich, PhD, from the Aligning Forces for Quality Evaluation Team based at Penn State University, presented the results of their recent consumer survey of the 14 Aligning Forces for Quality communities. The survey polled consumers with chronic conditions, such as diabetes. The poll was taken to gather baseline information on the communities. Questions fell into four categories: (1) patient care experiences; (2) self-reported receipt of evidence based care; (3) engagement in own care; and, (4) exposure to and use of public reporting. In three years, a follow-up survey will be conducted to assess the effects the Aligning Forces for Quality collaboratives have had in their respective regions.

My Health Counts – Shows 1 and 2–Pam Johnson, PhD, from WNED was present to report on the progress of “My Health Counts”. The first show, “Partnering with Your Doctor,” was previewed by approximately 44 people on Monday, November 3, 2008. The program will be broadcast on Tuesday, November 18 at 8pm on WNED-TV, and Monday, November 24 at 9pm on ThinkBright TV. The website will be launched with the broadcast. The topic for Show 2 is Self Management. A form was passed out requesting suggested people to interview who have been successful in self-management of their chronic condition. This form is available on Direct any questions to program producer, Christy May.
