Cargo heating log in AET format scanned and sent on completion of voyage.

D. Owner Specific Major Problems/ Incidents
1 / Bunga Kelana Dua grounded whilst making final approaches to the anchoring position in Johor port limits in June 2010. She had noticed a tug & tow drop anchor in the immediate vicinity leading to a close quarter situation. Drastic measures were taken to avoid contact with this tug & tow resulting in the vessel drifting away into No-Go areas and subsequently grounding. The main engine was run full astern and vessel refloated and later anchored safely. There was no passage plan prepared for shifting anchorage. The vessel’s position was not closely monitored duringshifting. OOW did not inform Master immediately about vessel entering NO GO area evenafter plotting the position. Master tried to hide the entire grounding incident from the office.
2 / Bunga Kelana Dua collided with another vessel which was crossing from the port side about 13 miles north east of the Eastern entrance to Singapore straits TSS in June 2010. Vessel was on regular short passages through the Malacca straits between ports in Malaysia and Indonesia.The OOW acquired and monitored a target showing a CPA of less than a mile, nearly two hours prior to the actual collision.Attempts were made to raise the vessel on VHF. These failed and the OOW altered own vessel’s course almost parallel to the other vessel. After Master took over the con, he became increasingly concerned about a crossing coast guard vessel. Master’s action to alter to port to avoid collision with the CG Mellon drew own vessel closer to the M.V. Honesty Ocean eventually leading to a collision. Own vessel failed to comply with Colregs. Cost: USD 450,000/-
3 / Eagle Augusta had a complete auxiliary engine no.3 breakdown while loading during an STS operation at Galveston lightering zone in April 2010. A/E 2 and A/E 3 were on load when the mooring winches and some other equipment were started. As a result A/E 1 started to share the extra load. The noise and vibration pattern increased abnormally in the Engine Room. The 3rd Engineer tried stopping the generator A/E 3 from remote buttons but the generator did not stop. Then he tried stopping by operating the overspeed trip device but still A/E kept running. The generator was finally stopped after shutting its main fuel valve. The crankcase was opened up and No.1 unit’s crankpin was found with deep indentations. The investigation revealed sluggish/ sticky fuel racks of some of the fuel pumps. Please ensure that overspeed trip device of auxiliary engines is tested as per company procedures. The engineers must be familiar with the location of fuel valves. Cost: USD 500,000/-

  • Kindly be reminded to ensure compliance with BW exchange and BW chlorination regulations for Cholera prevention,
  • Please note that this is very high risk area for Sea Borne Drug Smuggling and be on alert accordingly.
  • Kindly beware in case of transiting River Plate that in certain sections the banks shift continually and buoys may drift out of position or the buoyage may not yet have been re-aligned to the navigable channel, for which the most up-to-date information must be obtained from the local Agent or the Pilot authority in good time before the transit and extra care is to be taken when navigating in suspect areas.
  • Kindly be warned that exorbitant fines are imposed on ships for any misdeclaration of bunkers, lubes, paints, spares, equipment stores, cargo or crew's personal effects. The Customs or other authorities will look for the slightest excuse, even minor, innocent, discrepancies or mistakes, to levy huge fines (USD20-30,000 upwards) and/or extort underhanded payments. Please, therefore, ensure that all port papers and customs declarations are prepared carefully and accurately, all items and quantities are diligently accounted for and declared according to local regulations and requirements, to be ascertained by you from port agent early to allow you sufficient time to ensure proper compliance.
