Job Title: Senior Practice Nurse

Responsible To: Partnership

Location: Two Rivers Medical Centre

Accountable to: Management Team

Job Summary

In conjunction with other members of the Two Rivers Medical Centre Clinical Team, care for practice patients, both in the community and at the surgery, providing a comprehensive service including treatment, preventative care, screening and patient education.

The Senior Practice Nurse is responsible for the overall co-ordination of Practice nursing services and the nurse team; including the Healthcare Assistants. The nursing service is complimentary to that offered by the GPs so team working and a commitment to deliver high quality evidence-based care is essential.

General Consideration - It is not intended that all nurses undertake all types of clinics but that workload is shared with respect to general duties and that each nurse has other specific areas of responsibility. It is also an important part of the role to assist in the development of all the Health Care team.

1. Clinical Nursing and Treatment

·  Provide nursing treatments in participation with GPs or independently within agreed protocols.

·  Involvement in Chronic Disease Management

·  Wound care Management

·  Ear Syringing

·  Suture Removal

·  Assess & treat warts, verrucas & other skin conditions

·  Treatment of minor injuries (e.g. removal of splinters, dressing of minor cuts and burns etc.)

·  Vaccinations and other routine injections e.g. b12, zoladex, clopixol etc.

2. Preventative Care, Screening and Education

·  Provide general and specific health screenings to the practice patients including housebound patients (within agreed protocols).

·  Vaccination for: Children; Tetanus and Other Immunisations; Holiday Vaccinations.

·  Providing service for investigations for practice patients.

·  This includes: Phlebotomy; ECG; Spirometry; Blood Pressure, Doppler Assessment & STI Screening.

·  Providing service for cervical smears – within agreed protocols. Provide information to patient’s about screening services e.g. mammograms.

·  Providing family planning advice – within agreed protocols.

·  Involvement in patient education.

·  Eg: Asthma/COPD; Coronary Prevention; Dietary Advice. Reviewing & initiating suitable treatments & medications, developing action plans & when appropriate creating or recommending referrals.

3. Administrative

·  Maintenance of clear, accurate and contemporaneous notes along with essential data entered on computer record.

·  Ensure maintenance of equipment and stock levels of drugs, dressings etc.

·  Maintenance and monitoring of patient recall systems, for example, to cervical smear, vaccination and diabetic clinics.

·  Collection and maintenance of statistical information and administrative documentation relating to patients within the practice.

·  Attendance at such meetings within the practice as is deemed necessary.

·  Ensure PGD’s are current & signed.

·  Ensure Registration – legal status is upheld annually.

·  Ability to make appropriate referrals

·  Extended role for monitoring & advise the HCA team on ordering of vaccinations for surgery use.

4. Research and Audit

·  Participate as required in any research projects and audits within the practice.

·  Involvement in teaching of new practice nurses and pre-reg nurses as required.

·  Involvement in the preparation and review of protocols within the practice.

5. Communication

·  Communication is seen as an essential part of the practice nurse’s role. This includes communication with:

·  The Doctors

·  The Admin & Reception Staff

·  Other Members of the PHCT (District Nurses, Midwives etc.)

·  Hospital Departments

·  Practice Nurse Colleagues

·  Practice Manager

·  The Patient and their Relatives/Carers

·  There must be an appropriate regard for confidentiality within guidelines agreed by Professional Nursing Organisations, NMC and within the practice.

·  Adherence to data protection.

6. Professional Development

·  Maintain continued education by attendance at appropriate courses and study days as agreed with the Practice Manager and GPs.

·  Educational meetings as arranged within the practice.

·  Maintaining portfolio to reflect my professional development to the standard required by the NMC for continued professional registration purposes.

7. Infection Control

·  Applies infection control measures within the practice. This includes universal hygiene precautions, the collection and handling of laboratory specimens, segregation and disposal of waste materials, decontamination of instruments and clinical equipment, reporting and treatment of sharps injuries and dealing with blood and fluid spillage.

·  Educates patients and colleagues in adopting sound infection control measures

8. Emergency Management

·  Is competent in anaphylaxis and resuscitation

9. Clinical Leadership

·  Identifies and supports clinical audit and the monitoring of standards of care.

·  Identifies and supports practice development plans

·  Identifies, monitors and maintains the consistency of professional standards.

·  Takes appropriate action if standards are not met.

·  Recognises the development needs of others and takes appropriate action

·  Provides effective and dynamic leadership and management skills by organising and leading a multi-disciplinary team.

·  As a clinical expert is able to advise on educational opportunities that will ensure the development of the clinical practice.

10. Team Leadership

·  Manages or supports relevant staff to deliver a holistic approach to patient services.

·  Manages the efficient utilisation of the nursing resources.

·  Develops clinical protocols and guidelines for the clinical team to follow.

·  Ensures clinical supervision is in place for nursing staff.

·  Perform staff appraisals

·  Perform regular monthly one to one meetings with individuals of your team. Assess/address any points raised.

Confidentiality

·  The data must be held under the terms of the Data Protection Act and binding its principals.

·  Any matters of a confidential nature in particular any information relating to patients must under no circumstances be divulged or passed onto any unauthorised person or persons.

·  In the performance of the duties outlined in the Job Description, the post-holder will have access to information relating to the business. All such information from any source is to be regarded as strictly confidential.
