Comments must be made for items marked, "Needs Improvement." Complete in triplicate. Original is for Human Resources, one copy for the employee, and one for the supervisor. Evaluation is to be completed by May 1. Conferences should be held for any employee requesting one, or for any person with a performance concern or at the discretion of the supervisor.
