How can I help support the clinic?

In order to bring our vision to life, we need everyone’s help. You can help us by giving your time, talent or treasure. If you would like for us to contact you to discuss ways you might be able to help us, please list your contact information below.

Financial contributors are always welcomed and needed.

To be eligible for free medical services you must be a resident of Orangeburg or Calhoun County, have no health insurance, and live at or below 200% of the Federal Poverty Guidelines.

What services are available?

Physician, Nurse Practitioner and Nursing Care, Medication Assistance, Education, Health Promotion and Screenings.

Specialty Care Referrals: General Surgery, Orthopedics, Neurology, Pulmonology, GYN.

How do I apply?

You must first come to the clinic and fill out an intake form. If you are eligible and we can meet your medical needs you will be given the next

available appointment.

You MUST bring the following items. Failure to do so may result in delay of appointment.

Ø  A current, valid photo ID

Ø  Your Social Security Card

Ø  Proof of residency, utility bill, cable bill, property tax bill, home owners or renters insurance. If you are staying with a friend or relative, you need to bring proof of their residency and complete our No Income Support Verification Form.

Ø  The medicines that you are now taking.

Ø  30 days of current pay stubs and 1040 form for ALL EMPLOYED ADULTS over age 18 IN YOUR HOUSEHOLD.

Ø  Proof of SS, SSI disability, unemployment, and food stamps

Ø  Denial letter from DSS, Medicaid or Medicare. SSI disability or award letter.

Ø  Unemployment checks statements for a month.

Ø  Proof of food stamps
