Opticianry Student Road to Academy Official Rules

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER.

Opticianry Student Road to Academy is sponsored by Transitions Optical, Inc. (sponsor). This program is governed by these official rules. By participating in the program, each nominated attendee agrees to abide by these official rules, including all eligibility requirements. The program is subject to all federal, state and local laws and regulations and is void where prohibited by law.

ELIGIBILITY
The Opticianry Road to Academy is open tofull time students of accredited opticianry schools in the US members of NFOS. Each school is limited to one attendee. Transitions Optical, Inc. has the right to verify the eligibility of each attendee.

TIMING

Each school will nominate one attendee to attend Transitions Academy no later than November 15, 2016: the nomination is submitted at Transitions.com/NFOS

The sponsor reserves the right to extend or shorten the nomination period at their sole discretion.

ATTENDEE CONFIRMATION
Attendee will be notifiedby November 30, 2016.

Each attendee must complete the Road to Academy registration process in the time indicated in their notification.

DETAILS OF TRANSITIONS ACADEMY OPTICIANRY STUDENT PROGRAM

For each attending student, Transitions Optical will cover the cost of Transitions Academy registration— some meals provided at some Transitions Academy events and a shared room (upgrade to private room at attendee’s expense available).

Terms and conditions may apply. Incidental expenses and all other costs and expenses which are not specifically listed as part of the attendance package in these official rules and which may be associated with the acceptance, receipt and use of all or any portion of the awarded trip are solely the responsibility of the respective attendee. ALL FEDERAL, STATE AND LOCAL TAXES ASSOCIATED WITH THE RECEIPT OR USE OF ANY PRIZE IS SOLELY THE RESPONSIBILITY OF THE WINNER.

COMPLIANCE WITH LAWS. Transitions Optical and Attendees covenant and agree that they will comply with all applicable laws and regulations, including, but not limited to, the federal health care program anti-kickback statute, 42 U.S.C. §1320a-7b(b) (the "Anti-Kickback Statute"), and its state law counterparts. Transitions Optical and Attendees further covenant and agree that participation in the Contest shall not create any obligation on the part of Attendees or Transitions Optical to (a) purchase, order, or lease any items or services made or provided by Transitions Optical that may be paid for, in whole or in part, by a federal health care program or (b) recommend or arrange for the purchase, order, or lease of any items or services made or provided by Transitions Optical that may be paid for, in whole or in part, by a federal health care program. The term "federal health care program" is defined at 42 C.F.R. §1320a-7b(f) and includes Medicare, Medicaid, TRICARE and certain other government funded health plans and programs.

ADDITIONAL LIMITATIONS
Attendance is non-transferable. No substitution or cash equivalent of prizes is permitted. The sponsor and its respective parent, affiliate and subsidiary companies, agents and representatives are not responsible for any typographical or other errors in the offer or administration of the contest, including, but not limited to, errors in any printing or posting of these official rules, the selection and announcement of any winner or the distribution of any prize. Any attempt to damage the content or operation of this program is unlawful and subject to possible legal action by the sponsor. Transitions Optical, Inc. reserves the right to terminate, suspend or amend the Opticianry Student Road to Academy, without notice, and for any reason, including, without limitation, if the sponsor determines that the program cannot be conducted as planned or should a virus, bug, tampering or unauthorized intervention, technical failure or other cause beyond the sponsor’s control corrupt the administration, security, fairness, integrity or proper play of the program. In the event any tampering or unauthorized intervention may have occurred, Transitions Optical, Inc. reserves the right to void suspect entries at issue. The sponsor and its respective parent, affiliate and subsidiary companies, agents and representatives, and any telephone network, service providers and package delivery companies, are not responsible for incorrect or inaccurate transcription of entry information, or for any human error, technical malfunction, lost or delayed data transmission, omission, interruption, deletion, line failure or malfunction of any telephone network, computer equipment or software, the inability to access any website or online service or any other error, human or otherwise.

PUBLICITY

By participating, each entrant grants the sponsor permission to use his/her name, likeness or comments for publicity purposes without payment of additional consideration, except where prohibited by law.
