Instructions:
Prior to the commencement of any release request, the applicant player must be registered within his/her appropriate association for the current year.
The parent or guardian of the applicant player must complete Section 1 of this form and deliver it to the President of the association from which the release is requested. Only the information included withinSection 1 of this form will be considered by the committee responsible for determining release requests. The association is expected to acknowledge all submitted release requests within 72 hours of receipt. Following acknowledgement of receipt, the associationmay either approve the request based on the information provided or determine that a formal meeting is necessary. If the association chooses to schedule a meeting, the President of the association from which the release is requestedmustcommunicate the date, time and locationso that all interested parties may meet to discuss the merits of the request. All release hearings must be held within ten business days of receipt of the request.
Should the release request be granted, the association is required to upload a clearly scanned copy of the release request form signed by the association to the applicant player's Sportzsoft registration as proof of his/her release. Should the release request be turned down, the association must complete Section 2 of this form stating the reason(s) for denying the request. At that point, the applicant player may determine whether it is appropriate to request an appeal of the association's decision through the OLA's Appeal Process. Applicant players should note that the grounds for requesting an appeal must be submitted to the appropriate zone/league registration contact in accordance with the Appeals Process, which can be found under OR10 of the OLA's Rules & Regulations document.
