Subject: Purchase of Medicines for the ICDS Projects of the District

Srinagar for the year 2014-15.

Sealed quotations affixed with revenue stamp worth Rs.05/= are invited from Registered /Reputed Dealers for supply of the various medicine items of standard quality manufactured after October,2014 as per annexure “A” to this notice, which should reach to the Office of the undersigned by or before 17-03-2015 Upto 4P.M. The quotations should be accompanied within CDR/FDR worth Rs.10,000/= ( Rs ten thousand ) pledged to the District Programme Officer ICDS Srinagar (Chairman Purchase Committee) without which no quotations shall be entertained. The quotations shall be opened in the Office chambers of the District Programme Officer ICDS, Srinagar on 18-03-2015 at 3 P.M in presence of the Members of the Purchase Committee and the quotationers who would like to remain present on the occasion. The quotationers should be a licence holder and duely registered with the Health and Medical Education Department. The Committee reserves the right to accept or reject any quotation without assigning any reason there off. The quotationers should enclose samples of each item with the quotation in a transparent container duly signed by the quotationer.

Instructions for Quotationers.

The Photostate copies of the following documents should be enclosed while submitting the quotations:

1. Copy of Registration Certificate (Form VAT-03)

2. Copy of Certificate of Registration from the Central

Sales Tax (Registration and turnover Rules 1957 form –

B)

3. Copy of Annual Turnover for the last two years.

4. Copy of Pan Card.

5. Copy of Income Tax Clearance certificate for the last

two years.

6. Copy of the latest VAT clearance in VAT 46 for the

assessing year.

7. Copy of Affidavit pledging that I / we have read all the

terms and conditions of the tender notice and agree to

accept the terms and conditions in toto.

8. Copy of Licence.

9. Copy of Registration certificate from the Health & Medical

Education Deptt.

Due to poor response the last date for receipt of the Quotations for the supply of various Medicine items invited vide this office Quotation No: DPO/ICDS/Sgr/Med/2015/7209-12,dt;12.03.2015 which was scheduled by or before 17.03.2015 upto 4 P.M is extended upto 23.03.2015 upto 11 A.M and shall be opened on the same date at 3 P.M. The other terms and conditions shall remain unchanged
