AGREEMENT

AN AGREEMENT BETWEEN

NWTX CONF. VIM COORDINATOR AND NWTX CONF. TEAM LEADER

The below signed agreement is confirmation that the NWTX Conference VIM Coordinator ______and theNWTX VIM Team Leader______, have reviewed the below policies and rules and that the Team Leader will complete the attached MISSION REGISTRATION FORMand send it to the NWTX Conference Officesix weeks prior to departing on the mission trip.

This agreement outlines the NWTX conference policies. Team Leader agrees to complete and authenticate to the NWTX Conference Office that the six below conference policies and rules will be completed before departure to the planned mission opportunity on ______.

Because the Northwest Texas Conference is responsible for the Team Leader and all Team Members the conference is making every attempt to ensure that the Team Leader, sending United Methodist Church, United Methodist District as well as the NWTX Conference have taken all prudent steps to ensure that the Northwest Conference VIM policies have been explained and will be enforced so the planned mission trip results in a positive experience for all concerned.

Six weeks before leaving on the mission trip the Team Leader will complete the attached MISSION REGISTRATION FORM and forward it to the Conference Office, 1401 Avenue M, Lubbock, Texas 79401-3939. This form will be sent to the attention of ______, at the NWTX Conference Office.

The above listed VIM Team Leader understands and agrees to comply with the below listed NWTX Conference policies and rules governing travel to a mission site as a Volunteer In Mission team.

1. Has your team been invited and will it be serving at an approved United Methodist site?

2. Have you served on a VIM team prior to signing-up to lead this NWTX Conference team?

3.Will all your team members be safe sanctuary trained and background checked before leaving?

4. Have you been VIM Team Leader Trained and possess a current certificate from NWTX Conference.?

5. Will you be in possession of Notification of Death & Medical/Accident insurance forms for team?

6. Will you and all your team members will be covered by Medical/Accident coverage insurance?

Team Leader has read and understands the six policies and agrees to forward a completed copy of the MISSION REGISTRATION FORM to the NWTX Conference Office six weeks before leaving and to update that list with changes that transpire before departure.
