POSITION DESCRIPTION

Position Title: Support PartnerDepartment: Support Services

Classification: Non-ExemptSupervision: Support Services Coordinator

SUMMARY: A Support Partner is responsible for the daily assistance with needs of people in services per the person’s Service Plan. Support Partners focus on assisting people in services to maintain their health and safety and independence where applicable. Support Partners provide services including: PersonalCare, Supported Community Connections, Home Maker support, and Respite.

Education/ExpeRIENCE:

  • Must have a G.E.D or high school diploma equivalent.
  • Must have one year of experience working with persons with developmental disabilities, or be recommended or requested by a family member.

ESSENTIAL FUNCTIONS:

  • Reliable, regular attendance and punctuality
  • Ability to understand and use appropriate grammar and vocabulary and communicate effectively, appropriately and professionally both verbally and in writing in the language understood by the person in services
  • Maintain professional on-going communication with people receiving services and families, both verbally and in writing,in person or through voice mail, email or by phone
  • Ability to work with other employees, departments and outside individuals in a positive, professional manner
  • Provide assistance to individual’s in services in accordance with Service Plan and daily Contact Note goals and tasks
  • Maintain a basic knowledge of the SLS service to be provided, Service Plan Limitations (i.e. units/hours per week or month), and billable vs. non-billable activities
  • Document progress towards Service Plan goals for in daily notes
  • Participate in and provide feedback at Individual Service and Support Plan Review and meetings
  • Completecompetency training with Team Leader or Coordinator as necessary
  • Complete and submit paperwork including but not limited to billing, contact notes, timecards, incident reports and physician’s orderswithin the required timelines
  • Must have a valid driver’s license and good driving record as approved by North Metro administration
  • Must have the ability to work independently

OTHER DUTIES:

  • Assist in development of weekly schedule
  • Attend workshops, in-services and training sessions as assigned by supervisor
  • Drive personal vehicle for activities and appointments
  • Maintenance of a flexible schedule to meet program participant needs. Hours will vary from week to week. Periodic early morning and later evening hours may be required.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

  • Ability to assist an individual who is using a wheelchair through pushing and pulling the wheelchair
  • Must be able to sit for up to 4 hours at a time, use a computer keyboard and mouse up to three hours at a time
  • Must frequently stand for periods of ½ hour throughout an 8-10 hour day
  • Ability to stoop, bend and reach above and across body
  • Ability to speak and hear clearly over the telephone
  • Lift 75 pounds using proper lifting techniques
  • Ability and willingness to safely lift, pivot, transfer and assist anindividual in services in a variety of settings, i.e., wheelchair to toilet, bed to floor, etc. that may be required frequently and/or occasionally to meet health and safety needs of people needing this assistance
  • Ability to utilize approved physical crisis intervention techniques to manage behavioral episodes of people needing this assistance
  • Ability to stand, pivot and transfer up to 75 pounds when transferring people in services as needed
  • Ability to bend at the hips and knees
  • Ability to lift/push/pull/carry various objects of 10 – 20 pounds frequently and 30 -40 pounds occasionally.
  • Ability to stand and walk on hard surfaces for periods of time up to one hour throughout and eight to ten hour day
  • Ability to reach to shoulder level and above
  • Use computer for periods of time up to one hour throughout an eight to ten hour day
  • Drive various distances, within the individual’s community, in all types of weather conditions
  • Able to withstand exposure to various weather elements

MATERIAL AND EQUIPMENT:

Pencils, pens, calculators, automobiles, vans, pagers, cellular phones, computer, typewriter, photocopies, fax machine, message machine, safety belts, household appliances (i.e., microwave, telephone, vacuum), light bulbs, show shovels, wheelchairs, wheelchair lifts, washer, dryer, adaptive equipment, bedrails, assist with medical equipment (Hoyer lift, etc.)
