GRANT APPLICATION GUIDELINES
THE FOLLOWING GUIDANCE NOTES SHOULD BE READ PRIOR TO COMPLETING AN APPLICATION FORM

Grant applications are considered by our trustees on individual merit. The applicants details are confidential will be known only to the Trustees of Leicestershire Kidney Patients Association.

Items to assist with dialysis such as drip stands are to be supported by the renal nurse. Other types of grant applications must come with an endorsement either from the renal social worker, or a renal nurse.

Cash grants can only be given up to a maximum value of £100. If the application is for help in purchasing a piece of household equipment then the equipment will be ordered by LKPA, at their cost or a subsidy might be given towards such a piece of equipment providing LKPA has agreed to this.

Note: Submission of an application unfortunately does not guarantee that it will be successful.

LKPA funding is, limited, so we may not be able to fund or fully fund your request, even if your application meets our criteria

Examples with the Committee will consider application for:

Items which will reduce suffering, improve quality of life and enhance patient comfort:
Examples: wheelchairs; bath lifts; replacement fridge; washing machine, cooker etc.
NOTE: All white goods can only be ordered by LKPA, including wheelchair, bath lift etc

In certain instance travel costs visiting patients (family) in hospital, long term (up to £100 maximum).

A one off donation in times of crisis (as described by patient and supported by renal nurse or renal social worker).

The Committee will NOT consider applications for:

Luxury items such as, musical instruments, dishwashers, sports club membership etc.

Central Heating

Holiday Dialysis, Travel Insurance, or holiday spending money

Travel to ongoing dialysis

Pilgrimages

Funeral expenses

Car repairs

Ongoing payment of loans, arrears, rentals and fuel bills

Home improvements

The applicant must complete all parts of the form and explain exactly what the grant is for.

The supporter (renal nurse of renal social worker), must attach details supporting the needs and condition of the patient; family make-up and circumstances. Estimates, quotes and invoices, where appropriate, must accompany the application. Where the patient is not currently on dialysis a medical statement should be included to identify the patient’s renal condition. Failure to provide any of this information may cause delays in processing the application.
