Notes:

(1)  Please complete all the details requested in this form. An incomplete form may cause unnecessary delay. Please remember that the submission of this form does not guarantee a place in the Madrasa.

(2)  A copy (no originals) of the child’s birth certificate must be attached with this form. Applications will not be accepted without it.

(3)  All applications received will be dealt with on a first come first serve basis. Catchment area and siblings at the Madrasa will also be taken into consideration. It is in your best interest to return this form fully completed to us as soon as possible. We have a limited number of places in each class. To ensure we receive this form, please ensure you deliver it by hand during Madrasa hours.

(4)  One form is required per child. Single forms for two or more children will not be considered.

(5)  Minimum age for enrolment is 5 years. If your child is younger you are still encouraged to apply for a place in advance.

(6)  After your form has been received and reviewed by us you will be contacted when a place comes up for consideration.

(7)  If your address or telephone contact details change before we contact you please notify us in writing.

(8)  Upon a place being offered parents will be required to attend a meeting with the principal.

(9)  We request your du’as and support for the success of the Madrasa.
