NORRISTOWN STATE HOSPITAL is soliciting bids for snow removal from the grounds of the Eastern Pennsylvania Psychiatric Institute (EPPI)

WORK STATEMENT

1OBJECTIVE

The contractor is to furnish equipment and operators, as needed, for snow removal from the grounds of Eastern Pennsylvania Psychiatric Institute, hereafter referred to as EPPI. This property is being managed by NorristownStateHospital, 1001 Sterigere Street, Norristown, PA 19401.

2PHASES

General: The contractor will be responsible for plowing and snow removal on all road surfaces, steps and sidewalks as indicated. All snow plowing will be provided upon a phone call from either the contract monitor or on-site contracted security personnel. All road surfaces must be plowedto limit the maximum accumulation of snow/ice not to exceed two inches (2”) in depth. Within two (2) hours of the end of the snowfall, all designated road surfaces must be cleared. All road surfaces must be cleared to a minimum eight foot (8’) to ten foot (10’) width. Snow must not be piled to interfere with emergency exits, site distances for public or private roads or in any manner that might impede emergency access to all buildings or obstruct sidewalks. All contract employees will be required to sign in/out. (See Technical Requirements for details)

Equipment Requirements: The contractor will be responsible to furnish and provide all equipment and operators. All trucks used for plowing must be a minimum of a

1-1/2 ton, single axle dump truck, spreader auger equipped. The truck should be equipped with a minimum 9’ wide power angled plow. The snow blower must be a minimum 8 HP capable of blowing a 28” path of snow on a single pass. All equipment must be in good working order. Vendor prices must be inclusive of all travel and fuel costs at the time of bid. The Commonwealth will not pay for hours required for maintenance or repairs of equipment.

3BACKGROUND

Site Access: Due to the facility being patrolled by contracted security services, access to the property is restricted. Therefore, the contractor must contact the Contract Monitor to make arrangements to provide services. Length of Agreement: The term of this agreement is anticipated to begin on December 1, 2013 through April 30, 2014.

Payment Provision: The vendor will be reimbursed only for services accepted by NorristownStateHospital.

The vendor shall be paid upon satisfactory completion of work performed and submission of an invoice on the vendor’s letterhead.

Site Visitation: All bidding contractors are required to visit the job site prior to submission of their bid. A Proof of Visit form must be signed and electronically attached to your bid. If this requirement is not met, your bid will be rejected. The site visit will be on the grounds of EPPI at a date and time determined by the Contract Monitor, Walter (Lee) Stull, Facility Maintenance Manager 3, Norristown State Hospital.

4TECHNICAL REQUIREMENTS

One hour or unit of service is defined as a single vehicle with an operator in the process of snow removal (plowing) on designated hospital road surface. One hour of snow blower activity includes an operator and the snow blower operating on the grounds and or sidewalks of EPPI. No payments will be made for time periods of required repairs or when the vehicles or equipment is parked and not in the process of snow removal. There will not be any payments made for transportation of equipment or labor to the EPPI. Each contractor employee must sign in/out, listing date and time, and provide some form of photo identification every time they enter and leave the site. Copies of the sign-in log will be compared against all invoices for payment.

With the exception of Building #3, the Boiler Plant and Security parking, the facility is not in use and not all parking areas are to be kept free of snow/ice – only those areas designated as part of this contract document (refer to attached site map) are to be treated.

The contractor shall be paid only for services actually accepted by the Commonwealth. The contractor agrees to the terms, conditions and attachments which are part of this agreement. This agreement is not valid or enforceable until fully executed, approved and delivered to the contractor.

It shall be understood and agreed that any quantities listed in the proposal are estimated only and may be increased or decreased in accordance with the actual normal requirements of the agency.

5GENERAL INFORMATION

The contractor is responsible to supply and maintain the good working condition of all necessary supplies and equipment.

The contractor is responsible to supply all staff necessary to ensure the successful completion of the agreement.

The contractor is responsible to carry all necessary insurance and liability coverage to ensure the successful completion of the agreement.
