Mary Schleeter is the CEO and co-founder of Aging Plan-it, a geriatric care management company serving the Sacramento region. She studied and received a master’s degree in Gerontology from San Francisco State University and has an undergraduate degree in Criminal Justice from California State University Sacramento. In addition, to running her own company, Mary is an Adjunct Professor in the Gerontology Department at American River College and a Certified Administrator for Residential Care Facilities for the Elderly (RCFE). She served two years as a Commissioner on the Adult and Aging Commission for Sacramento County. In her spare time Mary is a coach for “A Matter of Balance”, a fall prevention program underwritten/sponsored by University of California at Davis. Mary’s professional background and experience includes mental health and aging, hospice and geriatric care management. Prior to owning her own company, she worked for over 20 years in the corporate world of finance and specialized in fraud investigations. Mary is passionate about serving seniors and helping them meet their personal goals to improve quality of life and to age successfully

Mariann hasattended both our chapter meetings and the Oct. event

over the years and a member of CMSA for many years.

Dr. Mariann F. Cosby is the owner of MFC Consulting in northern California operating since 1991. She provides legal nurse consulting, life care planning, medical cost projections, bill analysis, case management, and expert witness services. She has extensive experience in clinical, management, teaching, and consulting positions in emergency nursing, pediatrics, geriatrics/public health, school nursing, and correctional healthcare. Dr. Cosby currently consults for California Correctional Health Care Services and is an Assistant Professor for California State University, Sacramento school nurse credential program.

Dr. Cosby is an active member of many professional organizations including Case Management Society of America (CMSA) since 2002. In 2006-2007 she served on the Sacramento CMSA Chapter Education Committee and assisted with the development of the education programs. Should she get your vote as a Director, she is looking forward to providing additional input and support to the chapter.

Tiffany Go, BSN, RN

Tiffany Go is a Registered Nurse and is currently the ACO Executive and the Director of Quality for California ACO. Tiffany has experience in ACO administration, compliance, care coordination, provider education, and ACO quality metrics collection and submission. Prior to be being involved in the world of Accountable Care Organizations, she worked as an operating room nurse and during her free time, volunteered in international medical missions such as Operation Smile and Rotaplast International. She then worked as an entry-level employee and worked her way up to the General Manager position for an EMR company. Tiffany also has experience in healthcare advertising and Utilization Management.

Dawn Taylor currently works as a Case Managerfor the outpatient Neurology Clinic at Matter VAMC. She has been a CMSA member since 2009, she has her CCM. She is retired from the United States Army Nurse Corp in 2016. I am interested in being a board member.

Carrie Waisley, MBA.Community Relations Manager for Rehab Without

Carrie has attended both our chapter meetings and the Oct. event as a

Vendor and is a Corporate Member of CMSA.

Carrie has a speech therapy and business background. She has been serving the needs of various sized healthcare organizations in California and Northern Nevada in various management capacities for most of her career journey with an emphasis on physical medicine & rehabilitation for the neuro-injured patient population. Currently, she is the Community Relations Manager for Rehab Without Walls, a post-acute neuro solution provider. With her collaborative work style, she would like to share her talent and experience in promoting the rewards and benefits of CMSA membership.

She believes that case management is a major contributor to the successful outcome of a patient in the various levels of rehabilitation. Treatment provider's must rely on a case manager's partnership to structure an appropriate plan of care for patients with optimal use of resources. Becoming a board member for CMSA, Carrie hopes to be instrumental in helping develop resources for case managers and treatment providers and strengthen partnerships to help navigate through the ever changing landscape of healthcare delivery.
