METHOD OF CALCULATING RISK RATING SCORE

RISK RATING THE TASK
SEVERITY / SCORE / LIKELIHOOD / SCORE / RISK RATING SCORE
Major (Death, amputation etc) / 3 / High / 3 / The score is calculated by multiplying Severity by Likelihood to give:
1 no action or low priority
2 low priority action
3 or 4 medium priority action
6 high priority action
9 immediate action which will probably mean stop the task immediately
Serious (fracture of large bones, permanent or long term disability) / 2 / Medium / 2
Slight (minor cuts, scratches, bruises, abrasions etc) / 1 / Low / 1
GUIDANCE ON EXPECTED ACTION COMPLETION TIMES
Apart from Immediate if it is impractical due to operational reasons to meet timescales then other temporary alternative arrangements must be put in place to reduce the risk / Low Priority Action – Actions to be completed within 3 months
Medium Priority Action – Actions to be completed within 1 month
High Priority Action – Actions to be completed within 2 weeks
Immediate Action – If actions can not be done immediately work must stop until actions are completed
