EXCEPTIONAL PERFORMANCE AWARD/ SUSTAINABLE PRACTICES

2018 NOMINATIONS

Nominations will only be accepted until
5:00 p.m. PST, Friday, December 15, 2017

Purpose

To recognize individuals, teams or organizations that have made outstanding contributions to promote sustainability in public works. Sustainability is accomplished by the efficient delivery of infrastructure in an environmentally and socially responsible way that ensures the best choices in the long term. Up to two awards may be presented annually in the following categories with one possible from each category: 1) Individual, 2) Program/Organization or Agency

Eligibility

Nominations may come from federal, state or local government; educational institutions; nonprofit institutions; and private sector organizations. To be considered, the person or program/organization or agency must be located within the geographic footprint of the APWA NorCal Chapter (Del Norte, Humboldt, Mendocino, Lake, Sonoma, Napa, Solano, Marin, Contra Costa, Alameda, and San Francisco counties). Members of the APWA Board of Directors are not eligible during the time they serve on the Board. APWA members who serve on the Center for Sustainability are not eligible during the time they serve in this capacity.

Previous recipients of this award are not eligible for future consideration for three years after the receipt of the award.

Criteria

Nominations will be evaluated on the eligibility as noted above and on the criteria as noted below, also taking into consideration how well the nomination is communicated. The nominated individual, team or organization should demonstrate excellence in the following criteria:

  Complexity and severity of the challenges

  Use of original/innovative/effective approaches

  Impact and long-term benefits to public works

Submission Instructions

1.  Save your submittal in PDF format and with the following file name parameters: “2018_Sustain_Category_CandidateChapterProgramName.PDF”

2.  Address the Supporting Documentation section of this form

3.  Do not submit letters of recommendation

4.  Your nomination package is limited to these 3 pieces of information: a 2-page maximum cover letter that identifies the accomplishments and attests to the nominee’s positive performance in the category nominated, PLUS a 10-page maximum narrative explaining the basis for the nomination, PLUS the completed nomination form. All elements of the nomination must be in 11-point type, 1 inch margins

5.  Provide color photographs (2) in high resolution JPEG format. These will be used for promotional purposes by the association. Photos are not counted as part of the evaluation. Photos need to be included in the same email submission but placed in a separate file

6.  Send this completed and renamed form, cover letter, narrative, and the photos in one email to

7.  Keep a copy as your submittal will not be returned

Completed submissions are due by 5:00 p.m. PST December 15, 2017

Supporting Documentation

Please provide supporting documentation to your nomination, adhering to the sequence below. Please conform to the page limits described.

  A cover letter (2-page maximum) describing how the nominee addressed and/or met sustainable goals which can be defined as follows: “Sustainability in public works means seeking a balanced approach for a vibrant community today and tomorrow, and it is accomplished by the efficient delivery of infrastructure in an environmentally and socially responsible way that ensures the best economic choice in the long term." The cover letter should identify the accomplishments and attest to the nominee’s positive performance in the category nominated.

  A narrative (10-page maximum) explaining how the nominee has addressed the categories of economy, community, and environment. The items listed below are provided as examples:
