All participants of the project must agree to this nomination being forwarded for consideration by the CQUniversity Excellence in Engagement Awards selection panel and be familiar with the Opal Awards for Excellence in Engagement Procedures and agree to abide by the conditions therein.

Please address selection criteria 1-4 below in one or two paragraphs each.

SELECTION CRITERIA:

1.  Briefly describe the engaged service learning activity.

2.  Outline the positive impact of the engaged service learning activity on all stakeholders (particularly the community).

3.  Describe the learning, reflection and growth by the participant/s through their involvement in this engaged service learning activity.

4.  Provide evidence of the direct connection of the activity to each participating student’s education or training with CQUniversity.

5.  Attach with this form, any supporting documentation by relevant community groups or organisations (see referee statements below).

REFEREE STATEMENTS (word.doc, pdf, jpg):

Please provide a signed statement, or other similar evidence, from at least two referees, confirming that the nominated project’s contribution is outstanding and exemplary of mutually beneficial engagement. Where possible, these referees should be external to CQUniversity.

This statement should also confirm the student/s involvement in volunteer service which aligns with their area of study or training with CQUniversity.

Any publications and other materials seen as supporting the Nomination can be included however such documentation will normally be retained by the selection panel.
