Condition Management Programme teams are responsible for the delivery of Condition Management, which is the health component of the Pathways to Work programme. This is a joint working initiative between Jobcentre Plus, on behalf of the Department for Work and Pensions, and the Department of Health. NHS Grampian is commissioned by Jobcentre Plus to provide the CMP for people in Grampian in receipt of Incapacity Benefit, and who are referred by specialist advisors to the service.

The CMP provides one-to-one programmes of supported self-help to clients; in order to address complex health related barriers, which are preventing a return to the workplace. This work is based on the broad principles of Cognitive Behavioural Therapy. The aim of the CMP is to increase an individual's understanding of their health condition, and to work with him/her to develop strategies to manage it in such a way as to optimise their capacity to have an improved quality of life, and increase their confidence in order to facilitate a return to work.

Three multi-disciplinary teams will provide the service across Grampian from a variety of different venues, including Jobcentre Plus offices, client's homes (when appropriate), and other community venues. Teams work closely with Jobcentre Plus colleagues and other employment focused services within the CHP's and local communities.
