1.Advanced Services (MURS/NMS)

Requirements

Prem 1 and 2 Forms

Pharmacy contractors must self-certify to NHS England that their consultation area meets the required standard before beginning to conduct MURs. The Premises Self-Certification Form (PREM1) and the form to apply to NHS England for PCT consent to conduct an MUR away from a Pharmacy (PREM2) can be downloaded here.

2.Payment Adjustments

2.1Switched Prescriptions

The onus is on pharmacy contractors to ensure exemption declarations are duly completed in compliance with their standard operating procedures (SOPs). NHS England Area Team would only on exceptional circumstances switch prescriptions for good cause. Pharmacy contractors would have to demonstrate evidence of good cause.Guidance on how to avoid prescription switching can be found on the PSNC Website requests to be send to:

2.2 Top-up Payments Guidance

Where the total monthly Establishment and/or Practice Payments in relation to prescriptions passed for pricing are less than the amount which would have been paid had the payments been calculated on a six monthly basis, contractors can claim for a ‘top up’ payment. 3.Monitoring

100 hour pharmacies please send the completed embedded monitoring forms to NHS England North East & Central London Area Team quoting your pharmacy details i.e. pharmacy name, address and ODS codes. Please send via email to

5.Waste Collections

Pharmacy unwanted medicines waste (Pharmaceutical Waste) collection for Barnet, Enfield & Haringey is managed by PHS, contact number is 0870 241 7491.Unwanted medicines collections are made on a quarterly basis in most occasions.

Pharmacy unwanted medicines waste (Pharmaceutical Waste) collection for Camden & Islington is managed by Cannon Hygiene, contact number is 0844 967 0682. Unwanted medicines collections aremade on a quarterly basis in most occasions.

Following the regulatory changes that took place in respect of the governance of controlled drugs (SI 2013/373) on 1 April 2013 the responsibilities of the Controlled Drugs Accountable Officer (CDAO) of the Primary Care Trust have moved to the CDAO(s) of NHS England. The following information sets out the new reporting arrangements for matters concerning controlled drugs in the NHS England (London Region).
