New Student Enrollment Checklist
Parent/Guardian,
Welcome to Pender County Schools! In order for the enrollment process to run smoothly your cooperation is appreciated. Attached you will find several forms that need to be completed prior to meeting with our staff to enroll your child. Please take the time to complete these forms accurately.
The following items must be obtained and forms completed before your child can be enrolled in a Pender County School.
· Withdrawal form from previous school (for in-county transfers)
· Birth Certificate or Mother’s Copy
· Proof of parental rights or custody (parent’s name listed on birth certificate or a court order granting custody). A custodial guardian must be present to enroll the student.
· Proof of your residence: a lease, contract or deed. If you are living in a home where you are not the owner or renter, you must complete the Residential Affidavit.
(This form must be signed in front of a Notary Public by the owner of the residence.)
· Affidavit of Transfer (This form must be signed in front of a Notary Public)
· Current photo ID of parent/guardian
You must also provide a copy of:
o Health Assessment (Kindergarten Only)
o Immunization record. If you do not have this, you should be able to obtain it from your child’s previous school or doctor. Staff will not be responsible for obtaining this for you.
o Your child’s Social Security Card (optional)
* Any unusual circumstances must be approved by Principal*
In addition, make sure you complete the forms in this packet:
o Educational Records Request form
o Pender County Student Information Form
o Medical Alert Form
o McKinney-Vento form
o Home Language Survey
o National School Lunch Program Form (even if you don’t think you qualify)
o Bus Transportation Form
o Swish Form (Elementary Only)
