Personnel Actions.
- The 5th Squadron, 117th Cavalry objectives for FY04 is eleven (11) OCS Candidates.
- Troop Commanders are required to use the OCS Qualification Checklist to screen potential candidates.
- Troop Commanders will submit names to Squadron, NLT 19 November 2003 during the Squadron Command and Staff.
- OCS Boards will be tentatively scheduled to begin at 1300 on 6 December 2003 at Fort Dix Building 3650 classroom 1. Orientation Day for potential OCS Candidates is 10 January 2004 at NGTC.

Officer Candidate School (OCS)
Applicants Name:______
Age. Requirement: 18-30, TAG waiver to 35, NGB waiver to 40. Required Documentation: birth certificate (copy), waivers if needed.
Citizenship. Requirement: must be a US citizen prior to commissioning. Required Documentation: birth certificate (included above), Social Security Card (copy). Verification of Naturalization memo if a naturalized citizen (do not copy the naturalization certificate).
GT Score.Requirement: GT of 110 or greater. Required Documentation: DA2-1 (copy) or actual test score print out (copy). Soldiers may retest to raise score.
SAT/ACT.Requirement: SAT of 850 or greater (ACT 19 or greater)within 10 years. Not needed if the applicant has a college degree. Required Documentation: Test score results (copy).
Education.Requirement: 60 college credits to enroll, 90 to graduate. Required Documentation: Original College Transcripts and copy of diploma if a degree holder.
Physical.Requirement: Pass a Chapter 2 physical before the first day of OCS. Medical Waivers are submitted thru NGB-ARS. Required Documentation: Approved physical including SF 88 and SF 93 (or DD 2807-1 and DD 2808), waivers if needed.
Height / Weight.Requirement: Meet the standards in AR 600-9. Required Documentation: Signed memo stating the applicant is within the AR 600-9 standard, include the height, weight, and body fat information. If tape test is needed, include the computer print out.
APFT.Requirement: Pass a record APFT within 12 months. Required Documentation: DA 705 APFT card (copy), include ht/wt data.
Security Clearance.Requirement: Secret clearance or interim secret clearance (must be within 10 years). Required Documentation: SF 86 and (2) finger print cards, electronic information on disk (EPSQ), or clearance verification memo.
Prior Training.Requirement: BCT and AIT, AIT not required for OCS Enlistment Option applicants. Required Documentation: DD 214 (copy), copy of OCS Enlistment contract if no AIT.
Recommendations.Requirement: Letters of recommendation from the company level commander, not required for OCS Enlistment Option applicants. Required Documentation: Letter of recommendation from company level commander.
Autobiography.Requirement: Typed autobiography including civilian and military experience. Required Documentation: Typed autobiography.
ATRRS Enrollment.Requirement: Applicant enrolled in OCS thru ATRRS. Required Documentation: ATRRS print out.
NGB 62.Requirement: The NGB 62 serves as the application for OCS. Required Documentation: NGB 62 (pages 1-3).
Selection Board.Requirement: Applicants must appear before a (3) member board at the battalion level. Required Documentation: Memo with the board results signed by the board president.
