Foster Blanket Agreement
I, ______(hereinafter referred to as FOSTER FAMILY), do this _____ day of ______, 20_12__, enter into this Foster Agreement and acknowledge receipt from ROTTEN ROTTIE RESCUE, INC. (hereinafter referred to as ROTTEN ROTTIE RESCUE) the following foster dog named, ______(hereinafter referred to as FOSTER DOG) into my home.I understand that this contract will be provided for, with the best of my ability, in following the terms and conditions below.
TERMS AND CONDITIONS
FOSTER FAMILY agrees to the following terms and provisions:
- _____ (initial) FOSTER FAMILY agrees to take the FOSTER DOG to aROTTEN ROTTIE RESCUE approved veterinarian, as scheduled by ROTTEN ROTTIE RESCUE, as such appointments are made. ROTTEN ROTTIE RESCUE keeps all FOSTER DOG(s) under veterinary care until they are adopted.
- _____ (initial) FOSTER FAMILY agrees to abide by the following terms in caring fortheFOSTER DOG:
- _____ (initial) The FOSTER FAMILY will provide a safe and loving home environment.
- _____ (initial) The FOSTER FAMILY will treat the FOSTER DOG as if they were one of their own pets.
- _____ (initial) The FOSTER FAMILY will provide humane treatment in accordance with all current and future state, county and local laws and ordinances where FOSTER FAMILY resides.
- _____ (initial) The FOSTER DOGwill not ride loose or be tethered/tied or chained in the back of an open-bed truck
- _____ (initial) The FOSTER DOG will not be allowed to roam free in populated or urban areas or in violation of any laws.
- _____ (initial) The FOSTER DOG will be leashed and handled by a responsible person or kept in a safe and secure enclosure and not allowed off leash at anytime outside a contained yard.
- _____ (initial) The FOSTER DOG will not be tethered or tied/chained as a means of confinement.
- _____ (initial) The FOSTER FAMILY warrants that the FOSTER DOG will not be contained solely by a hidden fence.
- _____ (initial) The FOSTER DOG(s) will not be solely an outside pet and FOSTER FAMILY agrees that when the FOSTER DOG is outside it shall be protected from the elementsand provided with adequate food, water and confinement.
- _____ (initial) The FOSTER FAMILY agrees to take the FOSTER DOG(s) to aROTTEN ROTTIE RESCUEapproved licensed veterinarian to provide the FOSTER DOG with necessary veterinary care upon emergency sickness, disease and/or injury. A list of ROTTEN ROTTIE RESCUE approved licensed veterinarians and emergency clinics will be given to the foster family. At no time is a FOSTER DOG to be taken to the FOSTER FAMILY’S personal vet with expected reimbursement without written approval of the President and Board of Directors of ROTTEN ROTTIE RESCUE.
- _____ (initial) The FOSTER FAMILY understands that there are costs involved in maintaining a FOSTER DOG(s) and agrees to assume full responsibility for a quality food as provided or recommended by ROTTEN ROTTIE RESCUE, clean and fresh water, flea and tick care as needed, grooming and basic health problems including administering any and all medications, vitamins or supplements as provided or recommended by ROTTEN ROTTIE RESCUE.
- _____ (initial) FOSTER FAMILY also agrees that they will discuss with their personal veterinarian about the fostering of a dog and to seek advice whether he/she recommends a six-month distemper/parvo booster, bordatella booster, tick and/or flea treatments and/or regular deworming schedule for the family's personal pets to prevent any disease transmission between personal pets and FOSTER DOG.If the FOSTER FAMILY decides NOT to take the extra precaution with additional boosters, tick and/orflea treatments and deworming schedules, ROTTEN ROTTIE RESCUE will not be responsible for the family’s personal pets if an outbreak should occur.
- _____ (initial) FOSTER FAMILY agrees that the FOSTER DOG(s) will reside in his/her home, will be kept as a household petand will not be used as a hunting or guard dog(s).
- _____ (initial) ROTTEN ROTTIE RESCUE makes no warranties or representations regarding the FOSTER DOG(s)’s health or temperament. It is agreed and understood by the FOSTER FAMILY that while every effort is made to provide an accurate history and assessment of a dog, ROTTEN ROTTIE RESCUE does not warranty any FOSTER DOG(s) regarding medical status, behavior or disposition. It is further agreed that environmental changes may effect and change the temperament of the FOSTER DOG(s) and that ROTTEN ROTTIE RESCUE has no liability or responsibility of any nature regarding defects with the FOSTER DOG(s), or injuries or damage to any person or property which may be caused by the FOSTER DOG (s). FOSTER FAMILY agrees to indemnify and hold harmless ROTTEN ROTTIE RESCUE, its members and representatives against any and all claims, known or unknown, now or hereafter arising in connection with this FOSTER DOG(s).
- _____ (initial) FOSTER FAMILY agrees that ROTTEN ROTTIE RESCUE is the sole and rightful owner to the FOSTER DOG(s) and consents to the examination of this FOSTER DOG(s) by ROTTEN ROTTIE RESCUE at any time and acknowledges that ROTTEN ROTTIE RESCUE has the right to reclaim the FOSTER DOG(s) at any time and for any reason without notification.
- _____ (initial) FOSTER FAMILY agrees that if for any reason, he/she cannot keep this FOSTER DOG(s); he/she shall return FOSTER DOG(s) to ROTTEN ROTTIE RESCUE. The FOSTER DOG(s) shall not be given away, sold, exchanged or adopted out without prior written consent of ROTTEN ROTTIE RESCUE and all completed paperwork for a legal adoption. FOSTER FAMILY understands that any friend or family member who wishes to take ownership of this FOSTER DOG(s) will be required to apply to ROTTEN ROTTIE RESCUE for the right to adopt and/or foster the FOSTER DOG.
- _____ (initial) It is agreed and understood by FOSTER FAMILY and ROTTEN ROTTIE RESCUE that this written agreement sets forth all promises, agreements, conditions, and understanding between them, oral or written, and that both parties have fully read and understood all of this agreement.
- _____ (initial) FOSTER FAMILY agrees to pay ROTTEN ROTTIE RESCUE for any and all expenses, including court costs and reasonable attorney’s fees, in enforcing the terms and provisions of this contract. Payment of these damages shall not excuse FOSTER FAMILY from returning the FOSTER DOG(s) to ROTTEN ROTTIE RESCUE.
- _____ (initial) FOSTER FAMILY will have first opportunity to adopt their FOSTER DOG, but agrees that this will done following the adoption procedures set forth by ROTTEN ROTTIE RESCUE and that the adoption is contingent upon approval of the adoption committee.
- _____ (initial) FOSTER FAMILY will nothousethe FOSTER DOG in an environment where illegalactivities are known or reasonably suspected to take place.
- _____ (initial) FOSTER FAMILY will not permit the FOSTER DOG to be under the control of any person or persons known toparticipate or reasonably suspected of participating in illegal activities.
- _____ (initial) FOSTER FAMILY agrees that the FOSTER HOME is subject to annual recertification by ROTTEN ROTTIE RESCUE, and that the FOSTER DOG may be removed if, in the judgment of the ROTTEN ROTTIE RESCUE Board of Directors, conditions in theFOSTER HOME no longer serve the best interest of the FOSTER DOG.
- _____ (initial) FOSTER FAMILY also agrees that ROTTEN ROTTIE RESCUE may require a recertification of the FOSTER HOME at any time and may remove the FOSTER DOG if this recertification indicates that, in the judgment of the ROTTEN ROTTIE RESCUE Board of Directors, conditions in theFOSTER HOME no longer serve the best interest of the FOSTER DOG.
- ______BY SIGNING THIS CONTRACT, I AGREE TO ALLOW AGENTS OF ROTTEN ROTTIE RESCUE TO ENTER MY PROPERTY FOR THE PURPOSE OF REMOVING THIS FOSTER DOG FROM MY CUSTODY IF I DO NOT COMPLY WITH THE ABOVE TERMS.
I have read and received a copy of, and hereby agree to abide by, ALL THE TERMS AND CONDITIONS of this Agreement.
