NEW EMPLOYEE DEVELOPMENT EVALUATION FORM

To be completed by the Mentor of the new employee upon 30 day evaluation.

Ø  Obeys all site rules and regulations

Ø  Seeks to promote Health, Safety & Environmental in all aspects of their job

Ø  Adheres to P2S’s Employee handbook

Ø  Wears PPE at all times where required

Ø  Gets involved in the STA process … discusses safety concerns

Ø  Ensures proper training where required

Ø  Ensures tools and equipment are used in a safe manner

Ø  Seeks to correct unsafe conditions when identified

Ø  Complies with Fall Prevention requirements

Ø  Promotes good Housekeeping practices

Ø  Maintains Back Safety awareness, ensuring proper lifting techniques

Ø  Promotes good Fire Prevention practices

Ø  Ensures properly facilitated permit is in place when required

Ø  Promotes proper barricade usage

Ø  Obeys danger signs/tags

Ø  Understands the importance of immediately reporting any incident

  The new employee has demonstrated sufficient craft skills to perform their job at his/her craft level.

Mentor comments:

  This new employee, upon their 30 day acclimation process, has demonstrated proficient Health, Safety & Environmental commitment.

  This new employee is required to re-attend P2S’s orientation training program and will continue to be mentored and will be re-evaluated in 30 days.
