NC DPI Behavior Support Section PRC 29 Suspension Report Instructions

Open the Excel spreadsheet and select the appropriate month from the tabs at the bottom of the spreadsheet (Ex: When entering January suspensions, choose the January tab, and submit by February 10th).

1. LEA / Enter the name of the reporting LEA or charter school
2. Month of / Click on the cell to the right of this field and choose the appropriate month from the dropdown menu.
3. Student / Enter the names of all students receiving PRC-029 services.
4. School/
Hours / Enter the name of the student’s school and the number of hours that school is in session each day (Ex: If the school day begins at 8:15 and ends at 3:45, there is a total of 7.5 school hours each day).
5. ISS-Days / Enter the total number of days each student spent in an In-School Suspension setting, if applicable.
6. Days Suspended / Enter the actual number of days the student was suspended for the reporting month, if applicable.
7. Days Absent / Enter the total number of days the student was absent for the month, if applicable (this includes excused and unexcused absences).
8. Days Attended / Enter the student’s total number of days in attendance at school.
9. School Days in Month / Enter the number of days in the month that school is in session. This is not the same as actual calendar days. For example, March may have 31 calendar days, but only 20 days in which school is in session.
10. Person Completing Form / Enter the name of the person completing the form.

Other areas of the document will automatically populate. If the total percentages show up in RED, then the numbers do not match. Example:

Days Suspended / Days Absent / Days Attended / Days in school Month
2 / 3 / 15 / 20

The total number of Days in School Month should equal the sum of Days Suspended, Days Absent, and Days Attended.

Important Information:

Suspension reports are due to the NC DPI Behavior Support Section by the 10th of each month.

It is mandatory that the document be password protected.

Spreadsheets will no longer be accepted if there is no password protection. The spreadsheet will be sent back to the sender with a reminder.
