As a Separated or Retired Member of the Tennessee National Guard, you are eligible to convert your current State Sponsored Life Insurance (SSLI) coverage into a Whole Life Policy before Age 65. How To Apply:
1. Fill out the application completely and sign and date at the bottom.
2. Complete, sign and date the Form 4067-G, Authorization Agreement (Bank Draft Form).
3. Attach a VOIDED check to your Authorization Agreement.
4. Write a check totaling the first month’s premium for all applications.
5. Mail all documents to Katie at NGATNIT.
