Folic Acid Promotion Grants Application

Overview:

The National Council on Folic Acidis offering Folic Acid Promotion Grants for local agencies working to increase awareness of folic acid for women of child bearing age. Nonprofit groups are encouraged to apply. Previous grantees are eligible. Grants will:

  • Be awarded at the following levels:
  • Four at $5,000, or
  • Two at $10,000
  • Be awarded based on strength of applicants’ useof CDC folic acid materialsand available audience research.
  • Be used for folic acid promotion efforts, such as placement of a public service announcement or use of brochures and health education materials.
  • Be used for any associated program implementation cost (i.e. staff time, postage, transportation, etc.).
  • Be awarded based on strength of applicants’ plan to reach regions of the country with higher rates of neural tube birth defects.
  • Be awarded based on strength of applicants’ plan to reach the key audience of the National Folic Acid Program, young women.

This request for proposal seeks innovative ways of using the currentfolic acid materials that can be used as models to be replicated nationally.

Timeline:

An activity using the Folic Acid Promotion Grant is expected to be conducted within 12 months of the initial grant award. The initial grant award is expected to be made in January2009. Funds will be dispersed upon award. A final report will be due at the close of this grant opportunity, December 31, 2009. All grant moneys must be spent by this time and no further grant installments will be made. Required reporting format will be shared at the time of grant award.

A committee of volunteers and SBA National office staff will review and evaluate all complete applications. All applicants will receive notification of the status of their application on or before December 20, 2008. At this time the successful applicant will be advised on the next steps of project development. Work on this grant is expected to take place betweenJanuary2009 and December 2009.

 Description of the project and potential outcomes, including estimated number of people reached, audiences served, and plan for reaching key audiences of the National Folic Acid Program.

Describe how the CDC folic acid materials and available audience research will be utilized in your project.

 Project budget and include other sources of support. Please note: The funds must be used for grant project expenses, including: transportation, A.V. rental, photo copy services, or other services needed to implement the project. Grant funds may not be used for food, equipment or durable products.

Lessons Learned Final Report

A final report of key lessons learned will be due at the close of this grant opportunity, December 31, 2009. All grant moneys must be spent by this time and no further grant installments will be made. Required reporting format will be shared at the time of grant award.
