QUALIFICATIONS : Health, Sociology or related degree and a professional qualification in Nursing, Social Work or Allied Health Professions

SALARY : £39,228 - £42,964

Job Summary

To lead the development of a national care framework for people living with Huntington’s disease that effectively integrates the health and social care elements of support. To lead the localisation of the framework across Scotland and establish a national network that will improve information sharing and knowledge transfer among practitioners supporting people living with the condition. To better define and make transparent the individual journeys of people living with HD and improve their experience of support. To enable the development of a national strategy for Huntington’s disease and collaborate with other third sector organisations working in the field of neuro-progressive conditions. To capture improvement through good data collection.

MAIN DUTIES AND RESPONSIBILITIES

1.  Work with a wide range of stakeholders, use the available evidence base and lead the development of a national care framework for Huntington’s disease in Scotland.

2.  Enable and empower individuals living with the condition to strongly influence and create the national framework.

3.  Hold a series of workshops and conferences that will provide a focus for the creation and development of the care framework.

4.  Hold annual conferences to raise awareness of the national framework, share learning, examine challenges and solutions, and examine how the framework is being implemented in each region of Scotland.

5.  Establish, lead and support a core group of stakeholders who will input into the development of the framework.

6.  Write and publish the national framework in a variety of formats, making it available to as many stakeholders as possible.

7.  On the basis of the national framework produce an audit tool that can be used to measure the quality of local services for people living with HD.

8.  Collaborate with, and share learning and participate in appropriate networks with other third sector and statutory agencies involved in the support of people living with Huntington’s disease.

9.  Establish relationships with the Scottish Government Clinical Priorities Team, National Neurological Advisory Group, National Services Division, The Alliance and the Neurological Alliance of Scotland and work in partnership to enable the development of the national framework.

10. Agree a structure for HD NET - a national network of excellence that will engage stakeholders in improving support for people living with HD.

11. Work with Health and Social Care Partnerships to improve local implementation of the national framework.

12. Establish localisation groups throughout Scotland who will take forward the implementation of the care framework at regional/local authority level.

13. Support the localisation groups to audit their own area using the national framework and develop an action plan for improvement.

14. Establish mechanisms for communicating the work of the national framework, for example, developing information for the SHA web site.

15. Produce reports for the Clinical Priorities Team and the Scottish Government outlining the progress of the national framework.

16. Work with the Quality Improvement Team to develop a set of outcome measures that will demonstrate the impact of the national care framework.
