NARRATIVE AND FINANCIAL REPORTING FORMS

Porticus is very interested in knowing the results of a project that we have funded. Although we are very pleased to know of success, we also want to know of disappointments. We think there is just as much to learn for the future from setbacks as from projects that go smoothly.

All completed reports should be emailed to .

Reporting deadlines

For a one-year grant, the final narrative and financial reports are due a month after the grant period ends.

If you have a multi-year grant please complete this format the end of each 12-month grant period, indicating that it is an interim narrative and financial report.

If you have been invited to apply for a continuing grant before the current grant project is complete, please expect to receive an application, along with a deadline date, via email.

Requesting a Grant Amendment

Porticus requires that all grant amendment requests be made in writing for review and approval. Types of grant amendments considered:

  • Budget change
  • Project plan change
  • Project extension (due to remaining funds past the grant period)

Your written grant amendment request should include the following information:

  • File Number of the Grant
  • Program funded and grant amount
  • Reason for the request
  • How much funds remain unused
  • How you expect the requested change will help in the success of the program
  • Expected date to complete the project
  • A revised budget detailing how unused funds will be used

The written request may be submitted to . A program officer will review your request and respond accordingly.
