Please provide the information requested as it relates to grants numbered xx-HMEP-xx-xx. Only HMEP funded activities are applicable unless otherwise state.

This form is divided into Planning and Training sections. Respond to the section as applicable based on the Planning or Training categories of the Grant Award from the SERC. Respond to all segments of each question, as applicable.
