Kit and Equipment Order Form
Please take the time to read the below. If the form is completed incorrectly it will be returned and the processing of your order delayed.
Whether ordering rugby balls or large quantities of clothing the order form must be used. Below is a description of the information required in each column of the form.
Brand:
Nike / Gilbert / RFDL
Code (RFDL Items Only): Each item is coded in the catalogue; please state the relevant code here.
Item Name:
This must be the exact name by which it is referred to in the catalogues / on the price lists.
Colour:
Check the catalogue to ensure that the colour you state is clear and correct.
e.g. Do not write ‘Blue’ if the garment is available in both ‘Sky’ and ‘Navy’. Please state the actual colour and NOT the code for Nike items.
Size:
Check each catalogue as to the sizes available. Please note that items such as balls, bibs, gum shields and socks etc. also require sizes.
Quantity:
State the number of that particular item that you require in the given size and colour.
Price:
The price of the item in the singular i.e. the cost as it appears in the catalogue / on the price list. (Please note that these prices are excluding VAT; the total with VAT added will be confirmed for you once stock has been checked.)

When ordering kit please use the prices in the RFDL catalogue, on the Nike price list and on the Gilbert RDO price list. If you are approached by clubs or individuals the RFDL and Nike prices still apply however, for Gilbert items it is important that they use the ‘Club List’: these prices are still discounted but not to the same extent as those on the ‘RDO List’. When you are ordering kit as part of a joint initiative between yourself and a club, the RDO prices may be used.
RFU Logo:
Please state ‘YES’ or ‘NO’: this ONLY applies to NIKE items.
The RFU logo is embroidered as standard onto the RFDL kit and CANNOT be added to Gilbert items.
Cost Including RFU Logo:
This is only applicable for Nike items on which you require the RFU logo to be embroidered.
Add £2.75 onto the price which you stated in the ‘Cost’ column. (The costs of the Nike items with the RFU logo included are also available for you on the Nike price list.)
Total:
The cost of the item multiplied by the quantity (‘Cost’ x ‘Quantity’) or in the case of Nike items with the RFU logo to be added (‘Cost Including RFU Logo’ x ‘Quantity’).

The top of the form requires you to state:
Delivery address:
Please do not state ‘as per the footer of my email address’ but ensure that the address is written in full.
Name of the person ordering the kit:
This is your name as opposed to the name of the person/club of where you would like the kit delivered.
Budget Code:
Please ensure that ALL parts of the budget code are given.
6451-7001 is NOT a complete code; it is missing the initial set of six digits which determines the type of order that it is e.g. 314410 for Uniforms and Kit, 390900 for Sundry etc. 314410-6451-7001 is a complete code. Please check that you are familiar with the codes that apply to your accounts.
